Hepatic Hydrothorax

Abstract: Hepatic hydrothorax is an important and difficult-to-manage complication of cirrhosis and portal hypertension. Here, we aimed to study its clinical features and natural history. Complete clinical data, including outcomes, were abstracted from hospital records of patients with cirrhosis and ascites admitted to University of Texas Southwestern University teaching hospitals from January 2001 to July 2012. Hepatic hydrothorax was diagnosed based on currently accepted clinical characteristics of the disease, includ… Show more

“…Its detection helps to estimate the severity of liver disease. Badillo et al even observed that patients with HH lived shorter than the Model End Stage Liver Disease score predicted [13]. Our study cohort showed a 12-month transplant-free survival rate of only 63 %.…”

“…Primarily, data were collected from the NIS and NRD through ICD-9 coding and given patient privacy, these codes could not be verified via patient charts; however, the validity of these codes has been determined in previous studies. 33 When making the diagnosis of HH, other factors such as infection, cardiac disease, and pulmonary disease should be evaluated; however, we are unable to review each patient's chart to assess for these possible comorbid conditions. Due to the cross-sectional analysis of this study, association between hospital outcomes and factors of interest could be determined; however, in order to determine causality, a prospective study would need to be completed.…”

“…2 Approximately 4% to 12% of patients with decompensated cirrhosis are affected by HH. 3 HH is typically a transudative, rightsided pleural effusion, which results from the accumulation of ascitic fluid in the pleural space. 4 Multiple factors contribute to the accumulation of fluid, including a porous diaphragm that allows ascitic fluid in the abdomen to translocate into the pleural space, hypoalbuminemia, and drainage from the lymphatic system.…”
