Abstract:Heparin-induced thrombocytopenia type II (HIT type II) is an immunoglobulin-mediated, drug-induced side effect for heparin-treated patients with thromboembolic complications. With an incidence of 1-3 %, mortality of 20 % it and permanent disability for another 20 % is a clinically relevant disorder. With heparin treatment or prophylaxis frequent platelet count monitoring is necessary. ZusammenfassungZiel und Methode: Vom 1.10.1999 bis 31.3.2000 wurden an der Chirurgischen Abteilung des LKH-Natters 302 Patienten und 523 Beine wegen primärer Varizen operiert. Anhand einer prospektiven Studie haben wir untersucht, welchen Einfluß die Verwendung der intraoperativen Blutleere beziehungsweise die postoperative Thromboseprophylaxe mit niedermolekularem Heparin (20 bzw. 40 mg Enoxaparin) auf die Frequenz an postoperativen Blutungskomplikationen haben. Ergebnis: An Beinen welche in Blutleere operiert wurden kam es signifikant häufiger zu Blutungskomplikationen (30% Hämatome und Suffusionen) als an Beinen, die ohne Blutleere operiert wurden (10%). Bei Patienten, die in der postoperativen Phase zusätzlich zur physikalischen Thromboseprophylaxe eine solche mit NMH erhielten, hatten wir 40% Blutungskomplikationen und Patienten ohne zusätzliche medikamentöse Thromboseprophylaxe nur 7% Hämatome und Suffusionen. Schlußfolgerung: Wir verwenden in der Varizenchirurgie an unserer Abteilung die intraoperative Blutleere nicht mehr und machen nur noch bei Risikopatienten eine Thromboseprophylaxe mit NMH.

In the course of reconstruction of peripheral arterial occlusion processes, two gelatin-coated Dacron grafts and one collagen-coated Dacron patch were implanted in a 52-year-old male patient. Eight days following low-dose heparinization (5 days prior to surgery, 3 days postoperatively) with unfractionated heparin, with no clinical symptoms present, a dramatic isolated thrombocyte depression occurred, from 212 Gpt/l prior to surgery to 14 Gpt/l on postoperative day 3. Laboratory tests verified an HIT type II [heparin-induced platelet aggregation assay (HIPAA) and ELISA]. Despite immediate discontinuation of heparin and commencement of an anticoagulant therapy with Revasc and Refludan, an 8-week thrombocyte depression occurred which was eliminated only temporarily by administration of gammaglobulin. The specific antibody tests turned out positive for more than 5 months. Having ruled out other causes of thrombocytopenia, we assume that the case presented was either due to an interaction not elucidated to date or triggered by the grafts (gelatin/collagen/Dacron). The manufacturers of the grafts have disputed a heparinoid action.
