Henoch-Scho[Combining Diaeresis]nlein Purpura in Adults

Abstract: Abstract. Henoch-Schönlein Purpura nephritis (HSPN) has been extensively studied in children but, its natural history in adults is much less known. A cohort of 250 adults suffering HSP was retrospectively analyzed for a median follow-up period of 14.8 yr. “…Pillebout et al described in a retrospective review that the clinical findings of adult HSP include a high prevalence of palpable purpura (96%) and gastrointestinal symptoms (48%) (6). “…It has been reported that adult patients with HSP are at increased risk of progression from significant renal involvement to end-stage renal failure. Eleven % of adult patients reached end-stage renal failure, and 13% showed severe renal failure (CCr <30mL/ min) at a median follow-up of 14.8 years (6).…”

“…Conversely, end stage renal disease is reported in up to 60% of adult HSP patients (3,5) . Nevertheless, most series studied selected patients whose disease exhibited renal involvement severe enough to warrant renal biopsy (4,6) . Others studies have also suggested that cutaneous ulcers are more frequent in adult patients (6) .…”
