Hemodynamics of the Reconstructed Gastric Tube During Esophagectomy: Assessment of Outcomes with Indocyanine Green Fluorescence

Abstract: The gastric tube was divisible into three zones according to the dominant arteries present in the greater curvature under ICG fluorescence. The left gastroepiploic artery was enhanced in a direction opposite that of physiological blood flow in all cases. The median period from initial enhancement of the root of the right gastroepiploic artery to the most cranial branch of the left gastroepiploic artery until perfusion up to the tip of the gastric tube did not differ significantly between the "good" and the "sp… Show more

“…Dissection is carried up to the greater curvature of the stomach, where the gastroepiploic vessels are clipped and divided. In most cases, the left and right gastroepiploic vessels are in continuity, but in some instances there may be no direct connection . The short gastric vessels are divided, as dissection continues to the proximal right gastroepiploic vessels.…”

“…Other examples of NIR-guided reconstruction are described for breast and esophageal cancer. [72][73][74][75] ECONOMIC CONSIDERATIONS NIR guidance has the potential of reducing healthcare expenditure if improved outcomes warrant replacement of current technologies at a reduced unit cost. Lymphoscintigraphy is the standard for detecting nodal metastasis, but it is invasive, false-negatives exceed 5 %, radiation is detected, and it is costly and time consuming.…”

“…A few small case series have identified a larger degree of conduit ischemia in patients who developed anastomotic complications [38][39][40][41]. The sensitivity of this method in predicting leakage, however, remains to be seen.…”
