Hemodynamics of the inferior mesenteric arterial circulation

Abstract: The purpose of this study was to determine the significance of collateral supply from the hypogastric arteries (HGAs) to the inferior mesenteric arterial (IMA) bed. Peak systolic "stump" IMA and radial arterial pressures were obtained before and after clamping the right HGA, the left HGA, both HGAs, the middle colic artery (MCA) only, or the MCA plus right HGA, MCA plus left HGA, and MCA plus both HGAs in patients with aortoiliac aneurysm or occlusive disease. Six patients (four with aneurysms and two with occ… Show more

“…As demonstrated by Iliopoulos et al [10], the superior mesenteric artery provides the dominant collateral supply to the IMA bed. In the acute setting, the contribution via branches of the IIAs is insignificant in patients with AAA or aortoiliac stenotic disease.…”

“…At the inferior mesenteric artery level, contradictory results are found from the literature with regard to the participation of this artery to the pelvic circulation (9,18,21). The median sacral artery is reported as a cause of endoleak after endovascular repair of aortic aneurysms (12) or as a collateral route to the occlusion of the common iliac artery (24).…”

“…If not enough collateral circulation exists, ischemia can occur in the colonic mucosa. 15 Most of the existing literature regarding endovascular repair of iliac vessels is in the setting of atherosclerotic disease. Emergency stenting cases are rare and limited to isolated case reports.…”
