Hemodynamic Parameters in the Assessment of Fluid Status in a Porcine Hemorrhage and Resuscitation Model

Abstract: Background Measuring fluid status during intraoperative hemorrhage is challenging, but detection and quantification of fluid overload is far more difficult. Using a porcine model of hemorrhage and over-resuscitation, it is hypothesized that centrally obtained hemodynamic parameters will predict volume status more accurately than peripherally obtained vital signs. Methods Eight anesthetized female pigs were hemorrhaged at 30 m… Show more

“…The primary findings of this study are: 1) common clinical signs and symptoms, JVP, edema, and SOB did not correlate with measured PCWP values and 2) thoracic bioimpedance, ZOE®, measurements did not correlate to measured PCWP values in those same patients. These data provide a look at how these recommended examination findings and a common non-invasive measurement provide insight into a patient’s PCWP, the gold standard for volume status [ 17 ].…”

“…The signal is then deconvoluted into two components with a fast Fourier transformation (FFT) into the frequency domain. The cardiac component (fundamental frequency of the pulse rate [f 0 ]) and harmonics of the cardiac component are used to derive a value that estimates pulmonary artery occlusion pressure (PAOP), the best clinical measure of intravascular volume ( 5 , 8 ). Signals were collected from COVID-19 patients to assess volume status and the need for invasive measurements of volume state.…”
