Hemodynamic Changes During Laparoscopic Gastroplasty in Morbidly Obese Patients

Abstract: When compared to nonobese patients our obese patients tolerated the pneumoperitoneum surprisingly well, without experiencing fall in cardiac output. The hemodynamic consequences of peritoneal insufflation seem to be different in obese and nonobese patients.

“…In the face of this, Forfori et al trivialize the studies of Nguyen et al [3] and Artuso et al [4], which both used TDCO as their test method to determine CO and the derived indices of cardiac index (CI) stroke volume SV, stroke index (SI) and systemic vascular resistance (SVR) and SVRI. Alleging equivalence to TDCO, the authors emphasize that their results for SV are similar to those of Dumont et al [5], which are, on the mean, 64 and 72 mL, respectively.…”

“…In spite of the reported untoward hemodynamic effects attributed to laparoscopy, 18 obese patients tolerate the pneumoperitoneum surprisingly well, without experiencing a fall in cardiac output. 21 Intraoperative analgesia was provided with aliquots of fentanyl. The fentanyl dose was possibly high in some short cases (especially short laparoscopies), resulting in postoperative ventilatory depression and delayed extubation in the PACU (eight vs two cases in laparoscopic vs OPG).…”

“…Based on abnormalities in the cardio-vascular status in patients with obesity, numbers of reports (Dumont et al, 1997), have proved that those patients have an increased tolerance to peritoneal insufflations compared with patients with normal BMI.…”
