Hemisection of a Molar Tooth in a Dog: An Alternative to Total Extraction

Abstract: Hemisection is a viable option to complete extraction in those situations where a portion of a tooth can still perform in health and function after the diseased portion has been removed. A case report is presented describing the hemisection of a mandibular first molar in a dog. Indications and contraindications for the procedure are outlined.

“…Hemisection is a viable alternative to extraction in those situations where a portion of a multi-rooted tooth has normal root canal structure and can still perform in health and function after the diseased portion has been removed (Swan et al 1991). As described earlier, the procedure involves removing the compromised tooth structure (root and part of the crown).…”

“…After appropriate preparation, a root filling is then placed in the residual root and the crown sealed. The process of root canal treatment removes the dental nerves, preserves a functional tooth segment and thus maintains chewing physiology (Swan et al 1991). Even if a treated tooth goes on to develop further resorption, the lack of nerves prevents recurrence of tooth pain permitting retention of the tooth until it becomes structurally unsound.…”

“…The tooth is sectioned starting at the furcation and advancing coronally. 1,2,79,80 If the goal is root resection, then the bulk of the crown of the tooth is preserved. 2 If the goal is tooth resection, hemisection, or trisection, then the tooth is sectioned through the crown vertically from the furcation.…”

“…2 If the goal is tooth resection, hemisection, or trisection, then the tooth is sectioned through the crown vertically from the furcation. 1,[79][80][81] For partial tooth resection and root resection, the diseased root is elevated and removed. 1,2,[79][80][81] Odontoplasty is performed in all procedures to smooth sharp edges and to remove overhangs and developmental ridges, leaving a smooth surface free of deformities and plaque-retentive areas.…”
