Hematopoietic Cell Transplantation, Version 2.2020, NCCN Clinical Practice Guidelines in Oncology

Abstract: Hematopoietic cell transplantation (HCT) involves the infusion of hematopoietic progenitor cells into patients with hematologic disorders with the goal of re-establishing normal hematopoietic and immune function. HCT is classified as autologous or allogeneic based on the origin of hematopoietic cells. Autologous HCT uses the patient’s own cells while allogeneic HCT uses hematopoietic cells from a human leukocyte antigen-compatible donor. Allogeneic HCT is a potentially curative treatment option for patients wi… Show more

“…Regarding the severity of GVHD, acute and chronic GVHD were graded according to the National Comprehensive Cancer Network, Version 2.2020. 12 For the analysis of follow-up time, the starting day (day 0) was the day of allogeneic hematopoietic stem cell infusion and the end day was the day of death from any cause or 31 May 2021, whichever came first. Cumulative incidence was calculated from day 0 to the day of AML relapse.…”

“…In this retrospective analysis of 7 pediatric patients undergoing single-dose 7 Gy TNI-based primary conditioning before allogeneic HCT, 6/7 children achieved sustained engraftment leading to long-term survival in this mixed cohort as evidenced by the stable OS and EFS plateaus reached after transplantation (OS 85.7% at LFU, median FU 9.1 years). Due to the lack of an identical sibling or unrelated donor, haploidentical HCT was performed, which is nowadays considered a standard of care conditioning regimen [ 10 ]. It is currently difficult to compare our long-term outcome results with previous published data due to small-sized series, heterogeneity of patients and underlying diseases, and different FU times.…”

“…However, the algorithm at this stage is not clear in the guidelines in steroid-refractory cGVHD. ECP is among the second-line treatment alternatives [1,43]. However, its use with other agents, its role of other in the presence of new agents and the results of combination with them are still unclear.…”
