Helium-oxygen improves Clinical Asthma Scores in children with acute bronchiolitis

Abstract: Inhaled helium-oxygen improves the overall respiratory status of children with acute RSV lower respiratory tract infection. In patients with mild-to-moderate bronchiolitis (Clinical Asthma Scores of <6), the beneficial effects of helium-oxygen were most pronounced in children with the greatest degree of respiratory compromise.

“…We found one RCT of the use of helium/oxygen in children in an ICU (23). The authors entered 18 patients admitted to an ICU with RSV bronchiolitis into a double-blind, randomized, crossover study in which each would receive 20 mins of exposure to a mixture of either air/oxygen or helium/oxygen (heliox) via a face mask.…”

“…However, neither study described heliox gas flows for mask or HFNC delivery systems. The only prospective study for the use of heliox in the treatment of acute bronchiolitis in spontaneously breathing children was performed by Hollman et al 14 Subsequent controlled trials demonstrated a reduction in R aw in severe acute status asthmaticus during heliox inhalation as well as improvement in efficacy of inhaled bronchodilators delivered during inhalation of heliox mixtures. 4 The beneficial effects of breathing heliox mixtures likely derive from reduction in air-flow resistance and restitution of non-turbulent gas flow in the airways.…”

“…Increasing the helium-to-oxygen ratio failed to significantly decrease ventilation or oxygenation in infants ventilated with bronchiolitis (Gross et al 2000). Non-intubated infants with severe bronchiolitis administered heliox (helium:oxygen, 70%:30%) had a significantly improved clinical score (Hollman et al 1998), respiratory rate, heart rate, arterial oxygen saturation and length of time spent in the intensive care unit (Martinon-Torres et al 2002).…”
