Nº 36 Octubre 2014Página 44 CLÍNICA Comparación entre diferentes procedimientos de actuación en las úlceras por presiónCompare between different methods of treatment of pressure ulcer *Schmindt Rio Valle, Jacqueline *García García, Inmaculada *Gázquez López, María RESUMENObjetivos: Describir los diferentes procedimientos de actuación en las úlceras por presión en coxis y talones y comparar los resultados obtenidos en la evolución de las úlceras en dos centro hospitalarios.Método: Estudio descriptivo. La recogida de datos se realizó durante tres meses, de mayo a julio de 2010, en dos centros hospitalarios, Hospital Público (HP) y Hospital Concertado (HC). Se realizó el seguimiento de 25 úlceras por presión (UPP), localizadas 11 en el coxis, y 14 en el talón. Participaron un total de 18 pacientes, 9 del HP y 9 del HC, todos ellos firmaron un consentimiento informado. Los datos analizados fueron el motivo de ingreso, escala de Barthel, de Pfeiffer, el Mini Nutritional Assessment y la evolución de las lesiones según la National Pressure Ulcer Advisory Panel, fotografías tomadas "in situ" y el Pressure Ulcer Scale for Heading (PUSH). Resultados:De los instrumentos utilizados para valorar la evolución de la UPP, el PUSH permite discernir mejor los cambios producidos en las lesiones. Existe variabilidad en la actuación de tratamiento de las UPP en ambos centros hospitalarios, a pesar de que existe un protocolo de actuación en estas lesiones. En el HP a través del PUSH se detecta una mejoría significativa en las lesiones de los talones. En el HP utilizando como método de valoración, el PUSH se encontró una evolución positiva estadísticamente significativa en las lesiones de los talones. Using ultrasound for this purpose is particularly convenient since Doppler measurements of blood flow at the heel tissues can be included in such an examination as well [23]. Establishing routine ultrasound scans of the heels might also lead to development of new ultrasoundbased risk assessment tools that are more specific for identifying susceptibility to heel ulcers, particularly considering the individual's internal foot anatomy.…”

The cracked heel is one of popular problems occurred within thai society. In this study, stick-formed preparation (ST) was developed for portability and convenient to use. The formula contained natural oil, coconut oil (C) or rice bran oil (R), for moisturizing purpose. It was found that 10% R giving a suitable hardness for ST. After addition of 5% salicylic acid (SA) in ST, its hardness was decrease. The modification of formula was performed to obtain a suitable hardness. The ability of moisture retaining (MR) from ST and commercial cream (CO) were evaluated at 32°C for 24 hr with 50% relative humidity, the result was shown that ST giving better MR than CO (P=0.0028). After 4 cycles of freeze-thaw stability study, the ST hardness was not significantly change (P>0.05). However, the amount of SA in ST from dissolution with Paddle over Disc method using acetate buffer pH 5.5 at 32°C was lower than in CO.
