Aims Heart failure (HF) affects an estimated 38 million people worldwide and is the leading cause of hospitalization among adults and the elderly. Evidence suggests that there may be regional and ethnic differences in the prevalence, outcomes and management of HF. The aim of this study was to understand the disease burden and treatment patterns of patients hospitalized for HF in multi-ethnic Malaysia. Methods and results A retrospective, non-interventional study was conducted utilizing 10 years of medical records from the National Heart Institute Malaysia (IJN) from 1 January 2009 to 31 December 2018. Of the 4739 patients in the IJN database, 3923 were eligible and were included in this analysis. The study recorded a high male prevalence (72.3%) with a mean age of 62.0 (±13.26) years. The 30-day and 1-year rehospitalization rate was 6.8% and 24.7%, respectively. In-hospital mortality was 7.2% with 27.0% due to cardiovascular causes and 14.2% non-cardiovascular causes. The 30-day and 1-year rehospitalization rates were significantly higher in patients with lower systolic blood pressure (SBP, P < 0.001 and P = 0.002), diastolic blood pressure (DBP, P < 0.001 and P = 0.017), sodium (P < 0.001 and P = 0.029) and estimated glomerular filtration rate (eGFR, P < 0.001 and P = 0.002) and higher urea (P < 0.001 for both), serum creatinine (P < 0.001 and P = 0.003), and uric acid (P < 0.001 for both), respectively. Risk of hospitalization within 1 year varied significantly by ethnicity and was relatively higher in Indian (28.3%), followed by Malay (24.4%) and Chinese (21.9%; P = 0.008). In-hospital mortality within 1-year post-index date was higher in patients with lower weight (P = 0.002), body mass index (P = 0.009), SBP (P < 0.001), DBP (P < 0.001), sodium (P < 0.001), eGFR (P < 0.001) and higher heart rate (P = 0.039), urea (P < 0.001), serum potassium (P = 0.038), serum creatinine (P < 0.001), and uric acid (P < 0.001). In-hospital mortality within 1-year post-index date was also higher in patients with severe or end-stage chronic kidney disease (CKD) compared with mild/moderate CKD (P < 0.001) and in patients with HF with reduced ejection fraction (HFrEF) compared with those with mid-range or preserved ejection fraction (P < 0.001). The most commonly prescribed HF medications at discharge were loop diuretics (89.2%), β-blockers (68.5%), mineralocorticoid receptor antagonists (56.2%), angiotensin-converting enzyme inhibitors (31.5%), and angiotensin receptor blockers (20.8%). Conclusions This study provides a greater understanding of the characteristics, treatment patterns, and outcome of hospitalized HF patients in a leading referral centre in Malaysia and will aid the implementation of meaningful interventions to improve patient outcome for HF patients.

Aim This study aimed to explore the experiences of readmitted heart failure (HF) patients towards managing their medical problems and to identify factors influencing their adherence to medications. Subject and methods A qualitative exploratory study was conducted among HF patients who were admitted to Penang General Hospital, Malaysia. A purposive sample of 15 male and 5 female patients of various ethnic groups were interviewed using a semi-structured interview guide. The interviews were audiotaped and transcribed verbatim for thematic content analysis. Results The interviewed patients were generally disturbed by the deterioration in physiological function due to HF. They were aware of the importance of taking HF medications and control of fluid intake but not sodium intake. However, the participants still needed more clarification on ways to reduce sodium intake and improve adherence to lifelong medications and diet. The respondents identified individualized patient self-care, disease, and medication counseling that empower patients to monitor their own body condition to enhance adherence. Barriers identified were limited communication with health care practitioners, lack of correct information about medications and HF, difficulty in remembering, complexity of regimens, and lack of compelling HF symptoms. Conclusion The present study has identified empowerment of patients in decision making as the main facilitator to improve adherence. These findings suggest that there is a need for doctors and pharmacists to broaden their involvement in exploring and shaping individual patient's beliefs regarding HF and its management in order to achieve successful treatment outcomes.

Cardiovascular disease is an important cause of morbidity and mortality in Malaysia. Heart Failure, the end stage of most diseases of the heart, is a common medical problem encountered in general practice and is an important cause of hospital admissions. The 1 st Clinical Practice Guidelines (CPG) in Heart Failure was published in 2000. Since then, there have been many new developments in this field. Thus the publication of this 2 nd edition is timely. This CPG was drawn up by a committee appointed by the National Heart Association of Malaysia, Ministry of Health and the Academy of Medicine. It comprises cardiologists and general physicians from the government and private sectors and the public Universities. Objectives: The objectives of this CPG are to assist the health care provider in: Preventing heart failure Reducing the morbidity associated with the condition and improving the quality of life of these patients Improving survival of patients with heart failure Process: Evidence was obtained by systematic review of current medical literature on Heart Failure using the usual search engines-PubMed and Ovid. International guidelines on Heart Failure were also studied. After much discussion, the draft was then drawn up by the members of the Expert Panel and submitted to the Technical Advisory Committee for Clinical Practice Guidelines, Ministry of Health Malaysia and key health personnel in the major hospitals of the Ministry of Health and the private sector for review and feedback. The level of recommendation and the grading of evidence used in this CPG was adapted from the American Heart Association and the European Society of Cardiology (pg VII). The evidence supporting the recommendation was graded as: A if the data was derived from multiple randomized clinical trials involving a large number of individuals or meta-analyses. B if the data was derived from a single randomized clinical trial or limited to non randomized clinical trials or observational data. C if the recommendation was based on consensus of expert opinion or case studies only. In certain conditions even though there are no clinical trials but where the practice is nevertheless recommended based on years of well supported clinical experience the evidence, is graded as C. An example, is anticoagulation in the presence of a large mobile left ventricular thrombus. The grades of recommendation was ranked as I, IIa, IIb or III as outlined in page VII.
