Heart Failure After Myocardial Infarction: Clinical Implications and Treatment

Abstract: Heart failure is a frequent complication of myocardial infarction. Several factors, such as recurrent myocardial ischemia, infarct size, ventricular remodeling, stunned myocardium, mechanical complications, and hibernating myocardium influence the appearance of left ventricular systolic dysfunction after myocardial infarction. Importantly, its presence increases the risk of death by at least 3-to 4-fold. The knowledge of the mechanisms and clinical features are essential for the diagnosis and treatment of left… Show more

“…LV remodelling occurs in a quite significant number of AMI patients and is associated with development of heart failure, cardiac arrhythmias and a poor survival rate, therefore early identification of this consequence of AMI is clinically very important. 3,4,20 The process of LV remodelling begins immediately after the onset of acute MI. Along with the changes of the infarct zone and non-infarct area of the LV, activation of the neurohumoral system after AMI plays an important role in post-infarct remodelling development.…”

“…This study reports the greater prevalence of heart failure in myocardial infarction patients. 9,14,[18][19][20] Higher incidence of heart failure and the mortality in myocardial infarction patients in Bundelkhand could be explained by the patients' late referral to emergency and cardiac care units, scared of diagnostic facilities, and limited access to cardiac care.…”

“…25 However, the true prevalence and relevance of diastolic dysfunction after MI remains to be elucidated. 26 LV systolic dysfunction in patients with stable coronary disease is attributed to myocardial ischemia or hibernation, or prior infarction. 27,28 HF during hospital stay should be distinguished from pre-existing HF reported by patients on admission, which was present in 2.6% of patients who developed HF in this study.…”
