Biomarkers to predict risk of liver fibrosis in subjects with nonalcoholic fatty liver disease, a common risk factor for hepatocellular carcinoma, would allow for early preventive interventions. We sought to characterize bile acid profiles associated with liver fibrosis in subjects from the community‐based Cameron County Hispanic Cohort, a population in South Texas with high rates of nonalcoholic fatty liver disease, liver fibrosis and hepatocellular carcinoma. Plasma bile acid levels were measured in 390 subjects. These subjects were screened with liver elastography, detecting significant liver fibrosis in 58 subjects and steatosis in 186 subjects. Unsupervised clustering of the bile acid profiles revealed five clusters that differed by liver fibrosis, liver steatosis, liver injury, age and gender, identifying these parameters as major determinants of circulating bile acid changes. Total bile acid levels were significantly higher in subjects with fibrosis, with chenodeoxycholic acid displaying the greatest increase among individual bile acids. The primary conjugated bile acids, glycocholic and glycochenodeoxycholic acids, displayed the strongest association with fibrosis by logistic regression. High lithocholic acid levels were strongly associated with advanced fibrosis. In contrast, deoxycholic acid and total unconjugated secondary bile acids were positively associated with steatosis, whereas relative glycoursodeoxycholic acid abundance was negatively associated. Milk and yogurt intake notably contributed to fibrosis‐associated bile acid changes. In addition, multiple families within the Firmicutes phylum, Prevotellaceae, and Bacteroides species in stool significantly correlated with fibrosis‐associated and steatosis‐associated bile acid parameters, suggesting that the gut microbiome contributes to bile acid changes in the context of liver disease. Conclusion: Circulating bile acid levels were markedly but differently changed in liver fibrosis and steatosis in a high‐risk Mexican‐American population.

Background: There is limited information on physical activity in marginalized older populations like that on the U.S.-Mexico border. This study aims to understand physical activity engagement among older Hispanics residing in two U.S.-Mexico Border counties. Methods: The International Physical Activity Questionnaire (IPAQ) was used to measure physical activity in El Paso and Cameron County, Texas. Physical activity levels were reported for vigorous, moderate, and walking met/mins. Adjusted and unadjusted modeling was conducted to determine county differences and sociodemographic covariates. Results: There were 784 participants and 92.9% were less than 65 years of age. El Paso participants reported a significantly greater natural log met/mins of vigorous (β = 1.34, p = 0.000) and walking (β = 0.331, p = 0.006). Significant sociodemographic covariates in El Paso for vigorous met/mins were gender (females β = −1.20, p = 0.003), having a regular doctor (β = −0.779, p = 0.029), and acculturation (β = 0.513, p = 0.019). Significant associations in Cameron County were having a regular doctor (β = −1.03, p = 0.000) and fair/poor health status (β = −0.475, p = 0.001). Conclusion: Level of physical activity may differ in older Hispanics by urban context on the U.S.-Mexico border. Future physical activity programs to promote physical activity should take context into consideration.

Objective To evaluate a community-wide campaign, Tu Salud ¡Si Cuenta! (TSSC), in improving eating behaviors and anthropometric outcomes in Hispanic border communities. Design A quasi-experimental study with matched intervention and comparison communities. Cross-sectional assessments with randomly sampled adults, examined by actual exposure and site (Unexposed Intervention, Exposed Intervention, Unexposed Comparison). Setting/Participants Predominately Mexican-Americans located in Brownsville, TX (intervention) and Laredo, TX (control). Intervention TSSC included TV and radio segments, community health worker (CHW) discussions, and newsletters delivered in Brownsville from 2005–2010. Main outcome measures Healthy and unhealthy eating indices and average hip and waist circumferences. Analysis Univariable and multivariable regression models. Results Sample (N=799; 400 comparison, 399 intervention) was 98% Mexican origin, and 54% completed grade 9 or higher. Exposure to any TSSC component was associated with a lower rate of unhealthy food consumption. As compared to the Unexposed Intervention group, the Exposed Intervention for the newsletter had a higher rate of healthy eating (adjusted rate ratio (RR) = 1.18, p<0.01). As compared to the Unexposed Intervention, the Exposed Intervention for the CHW discussion had a smaller hip circumference (adjusted mean difference=−5.77 cm, p<.05) and a smaller waist circumference (adjusted mean difference=−5.25 cm, p<.05). Conclusions and Implications This study provides evidence for the use of community-wide campaigns for nutrition and obesity-related outcomes in Hispanic communities.
