Various issues confront Japan's elderly population, particularly in rural areas. These issues need to be examined to fully understand the dynamics of the provision of health care. Only by understanding the different components and their impact on patient care can improvements be made to the delivery of services and ultimately the quality of care. In this review, a consideration of different aspects of care is undertaken, with a focus on rural areas. Potentials for improvements are also discussed. Electronic databases such as CiNii and ICHUSHI searched by Japanese and PubMed and CINAHL searched by English were searched to retrieve peer-reviewed primary research literature. A narrative synthesis of the findings sections of the papers was applied to identify key themes. These themes consider the range of issues affecting health care in rural Japan: Service delivery, distribution, resources, health care, rural and elderly. In total, eight papers with either qualitative or quantitative methodology were included in the review. The review highlights a broad range of issues being confronted, most of them likely to have a medium to long term impact on the quality of care in Japanese rural areas. Government, community and professional carers' attention to these issues is thus critical to ensuring improved health care to reassure a high quality of life for elderly in rural areas in Japan.

Objective: to identify the perspective of elderly persons on healthy aging as described by scientific literature. Method: a descriptive integrative review type study was performed, guided by the question: what knowledge has been produced about healthy aging from the perspective of the elderly? It was carried out using the Scopus Info Site (SCOPUS), Cumulative Index to Nursing & Allied Health Literature (CINAHL), Medical Literature Analysis and Retrieval System Online (MEDLINE), Literature of Latin America and the Caribbean (LILACS), EMBASE and WEB OF SCIENCE databases and in the directory of the Scientific Electronic Library Online Journals (SciELO), for literature published in the period between 2005 and 2016. Result: Eleven papers were regarded as eligible for this review. These studies revealed that healthy aging is related to different health dimensions: biological (adoption of healthy habits and behaviors with self-responsibility), psychological (feelings of optimism and happiness), spiritual (faith and religiosity) and social (reciprocity in social support factors and the capacity to live autonomously and independently). Conclusion: the synthesis of knowledge on healthy aging from the perspective of the elderly can support the actions of professionals who work with this population group to encourage and value the social determinants involved, so overcoming the exclusive focusing on the adoption of habits and behaviors inherent to lifestyle to achieve aging in a healthy way.

Salutogenesis introduces a paradigm that requires a perceptual change towards what creates health and how health can be facilitated. Removing the lens of pathogenesis, aging is an achievement to be embraced and older people are valued as assets for their wealth of experiences, resources, skills and knowledge. From the perspectives of older adults, the concept of healthy aging is multidimensional, comprising bio-psycho-social-spiritual health. Evidence shows that sense of coherence via resistance resources promotes health outcomes among older adults. However, very few works have attempted to operationalise the salutogenic theory to promote healthy aging among older community dwellers. This chapter provides a detailed description of the Salutogenic Healthy Aging Program Embracement (SHAPE) intervention for senior-only household dwellers. SHAPE represents an application of the salutogenic concepts: sense of coherence and resistance resources. SHAPE is an integrative person-centric multi-dimensional health resource program that employs an asset-based insight-oriented approach. Illustration of examples in which how the salutogenic concepts were operationalised in developing the SHAPE intervention approach, its content, activities and the conduction of the intervention are presented.
