Objectives: To provide a live-experience knowledge base about biophilic design parameters and environmental features to inform policy and design in clinical therapeutic environments. Background: It is increasingly important to review hospital design to make the best use of the affordances of natural elements in supporting both patients’ and staff’s physical and psychological well-being. The biophilic design theory provides an appropriate design approach. However, current biophilic design frameworks fail to provide efficiently standardized guidance. This systematic review aims to examine the experience of hospital users (patients and staff) with a view to informing a standardized biophilic design framework to improve future design in this context. Methods: This study performed a review and synthesis of nine studies identified using systematic procedures focusing on biophilic design features in healthcare environments. Results: The study identified a selection of biophilic parameters specifically relevant to this building typology, according to three different user groups: outpatients (fresh air, light-daylight, thermal comfort, welcoming and relaxing), inpatients (feeling relaxed and comfortable, prospect refuge, security and protection, light-daylight, view), and staff (privacy-refuge, quietness). Conclusions: The systematically identified studies helped to identify and rank the biophilic design parameters that appear the most critical for promoting and supporting human health and well-being in clinical therapeutic environments from the user’s perspective. It also provides an up-to-date compilation of crucial design interventions related to biophilic parameters and as such provides benchmark information for future research and design guidance in these environments.

Comparing Face-to-Face, Blended and Online Teaching Approaches for Practical Skill Acquisition: A Randomised Controlled Trial

Introduction The efficacy of blended and online teaching methods for practical skill acquisition remains ambiguous, particularly for skills requiring haptic awareness and/or sensory training. This study aims to compare three teaching methods (face-to-face, blended, online) for the acquisition of skills requiring sensory learning and haptic awareness. A secondary aim was to explore student experience of each teaching approach. Design A post-test only randomised controlled trial. Methods Forty-seven participants chose between learning two skills: manual measurement of blood pressure during exercise (BPM) and/or skin fold measurement using callipers (SKM). Participants were randomised to one of three learning groups: face-to-face (n = 23), blended (n = 22) and online (n = 26). Assessors determined skill competency during an in-person skill demonstration session. A survey captured student experiences. Results For SKM, there was a statistically significant difference in skill competency between the online learning group (17% achieved competency) and both the face-to-face (75% achieved competency; p = 0.011) and blended (89% achieved competency; p = 0.001) learning groups. For BPM, the online group had the lowest percentage of participants achieve overall skill competency. Both knowledge-based and sensory-based sub-competencies were negatively affected by the online learning method. For both skills, students in the face-to-face and blended learning group were significantly more confident in their knowledge and their ability to perform the skill in a clinical setting, compared to the online learning group. Conclusion Both face-to-face and blended teaching methods were more effective at leading to skill acquisition and were preferred by students when compared to a fully online teaching method.

The theory of supportive design influences healthcare facility design but is under-researched for different cultural groups. This mixed-methods study compared two Indigenous sample populations in Australia to examine the effect of the physical environment in public hospitals and clinics on Indigenous people’s perceptions and experiences of waiting for care. Quantitative survey data ( n = 602) measured perceptions of relevant design attributes using paired images in a screen-based survey. Semi-structured interviews ( n = 55) identified concerns about the physical healthcare environment including waiting rooms. Ceiling heights, seating arrangements and views to the outside were significant showing commonalities between perceptions of the two populations. The interviews revealed that cultural and social constructs, including privacy, fear, shame, and racism, were significant and that people’s perceptions were influenced by colonization and independent of location. Our study highlights the importance of a cross-cultural approach to supportive design interventions for spatial and symbolic treatments of waiting areas.
