With the onset of the COVID-19 outbreak, a limited healthcare force catered to an ever-increasing number of COVID-19 patients on top of non-COVID-19 patients. Over two years after the pandemic, it is evident that medical practice must adapt to COVID-19 to ensure the provision of undeterred, quality medical care. This review intends to re-evaluate the Tonsillectomy protocols that we employed during COVID-19 and explore ways in which we might improve the safety of this frequently performed surgery even after the pandemic. A decrease in the frequency of tonsillitis was observed as lockdown and strict social distancing protocols were followed. Thus, it is concluded that tonsillectomies may be deferred unless urgent and in the best interest of the patient and medical workforce. However, when surgery is necessary, new mitigation strategies, like preoperative COVID-19 screening, minimizing intra-operative staff, widespread use of personal protective equipment (PPE), negative pressure operating rooms, and reducing postoperative hospital stay need to be incorporated into the safety protocols of airway surgeries and minimize aerosolization such as using SES (smoke evacuation system) during surgery and TORS (trans-oral robotic surgery). This narrative review covers an extensive literature review of papers on PubMed, Google Scholar and Medline that highlight the impact of the Pandemic on the surgical practice in patients of recurrent tonsillitis since December 2019. Введение. По данным отечественной литературы, в структуре гнойно-воспалительных заболеваний глотки паратонзиллиты и паратонзиллярные абсцессы занимают 35–50% у пациентов трудоспособного возраста, у лиц старше 50 лет заболеваемость ниже, статистические показатели отличаются в регионах, а также у пациентов с различным иммунологическим профилем. Пандемия новой коронавирусной инфекции внесла свои особенности в оказание медицинской помощи. Поликлиническое звено было перегружено не только в России, но и в других странах мира. В этой связи своевременная и ранняя диагностика гнойно-воспалительных заболеваний глотки играет важную роль в выборе дальнейшей тактики лечения данной патологии. Цель. Выявить взаимосвязи между клиническими признаками у пациентов с ПТА. Повысить эффективность диагностики ПТА на догоспитальном этапе оказания медицинской помощи с использованием современных информационных технологий. Материалы и методы. Проведен ретроспективный обзор 102 историй болезни пациентов в возрасте от 18 до 45 лет, пролеченных с 2015 по 2020 г. на базе оториноларингологического отделения ФГБУ «426 военный госпиталь» Министерства обороны Российской Федерации. Результаты. Результаты проведенного исследования выявили совокупность признаков, характерных для ПТА. На основании полученных данных было разработано мобильное приложение на базе операционной системы Android «Программа диагностики гнойно-воспалительных заболеваний глотки». Заключение. Выявленные в исследовании клинические особенности паратонзиллярного абсцесса акцентируют внимание оториноларингологов на использовании опросников в качестве первичной диагностики и способствуют снижению затрат на догоспитальном этапе обследования. Introduction. Russian scientific literature data show that paratonsillitis and paratonsillar abscesses occupy 35–50% in the structure of purulent inflammatory diseases of the pharynx in patients of working age, the incidence is lower in individuals over 50 years old, statistical indicators differ in regions as well as in patients with different immunological profile. The pandemic of a new coronavirus infection has brought its own peculiarities into healthcare assistance. Polyclinics were overloaded not only in Russia, but also in other countries of the world. In this regard, timely and early diagnosis of purulent inflammatory diseases of the pharynx plays an important role in selecting further treatment tactics of this pathology. Purpose. To identify relationships between clinical signs in patients with PTA. To improve the effectiveness of PTA diagnosis at the pre-hospital stage of medical care using modern informational technologies. Materials and methods. A retrospective review of 102 case histories of patients aged between 18 and 45 years treated from 2015 to 2020 at the otorhinolaryngological department of the Federal State Budgetary Institution "426 Military Hospital" of the Ministry of Defense of the Russian Federation was conducted. Results. The results of the study revealed a set of signs characteristic of PTA. Based on the data obtained, a mobile application based on the Android operating system "Diagnostic program for purulent-inflammatory diseases of the pharynx" was elaborated. Conclusion. The clinical features of paratonsillar abscess revealed in the study emphasize the use of questionnaires by otorhinolaryngologists as a primary diagnostic tool and contribute to reducing costs at the pre-hospital stage of examination.

Retrospective analysis of the prevalence of acute respiratory infections in children of the Ivano-Frankivsk region

Background. The article presents modern data about the prevalence and peculiarities of the course of acute respiratory viral infections in a childhood based on retrospective analysis of medical records. The purpose of this article was to analyze the epidemiology and features of acute respiratory viral infections in children based on the retrospective analysis of medical records, and reports of the Public Health Center. Materials and methods. Extracts from the medical charts of inpatients with acute respiratory pathology and statistical reports of the work of the ENT department of Ivano-Frankivsk Regional Children’s Clinical Hospital for 2017–2022, which included 1,177 patients, were carefully studied. Results. The clinical features of acute respiratory infections in children hospitalized to the ENT department were as follows: age over 3 years (89 %), frequent admissions to the hospital (> 4 times a year) at the age of 4–5 years, nasal congestion (87.0 %), headache (72.0 %), body temperature above 37.9 °С (37 %), purulent nasal discharge (20 %), cough (15.6 %), general weakness (12.3 %). The average duration of the disease before hospitalization was 8.0 ± 0.2 days, and after hospitalization, it was 7.0 ± 0.1 days. It was found that those hospitalized had mostly ENT pathology, which initially was more likely to be viral (75.0 %); moreover, predominantly girls were affected (45 %), χ2 = 15.7, p < 0.05. At the same time, the proportion of combined forms of ENT diseases with a complicated course was 25 %, and boys were more likely to be affected (60 %), χ2 = 18.9, p < 0.05. Pansinusitis, on the other hand, occurred more often against the background of secondary chronic pathology (70.0 and 30.0 %, p < 0.05), χ2 = 32, p < 0.05. During all years of observation, the age group of 3 years and older prevailed among hospitalized children with acute respiratory infections. Most patients were admitted to the hospital in a condition of moderate severity. In all children hospitalized during the reporting period, we have found signs of respiratory syndrome such as nasal congestion and anosmia, sore throat, cough, purulent nasal discharge, as well as symptoms of intoxication: general weakness, headache, fever. Conclusions. The obtained data require further study and observation in the dynamics in order to stratify risk factors for the development of IgA-vasculitis in a childhood.
