Background There is an aging population all over the world, and Saudi Arabia is no exception to it. An aging population poses several challenges to the health-care sector. Aim The aim of this study was to examine the health status and functional abilities of elderly people visiting primary health-care centers in Khamis Mushait, Saudi Arabia. Methods This cross-sectional study involved elderly male Saudi nationals aged 60 years and above. Self-rated health status and body pain, functional status, geriatric review of systems and other relevant data were collected using the Geriatric Health Questionnaire of University of Iowa Health Care. All statistical analyses were performed using SPSS version 21. Results The study results showed that the overall health of the elderly was either good (32.9%) or very good (32.3%), while 14.2% rated their health as excellent. Only two participants rated their health as poor. The majority of the participants (71.9%) had no functional impairment in basic activities of daily living (BADL), while 35.7% had mild impairment in instrumental activities of daily living (IADL). Only two participants were found to have total impairment in IADL. The most common limitation among BADL was walking (21.1%) and among IADL was doing the housework. The notable factors associated with functional impairment in BADL and IADL were old age (≥80 years), poor eyesight, memory impairment, having trouble with control of bladder, falls twice or more, taking more than two medications, being sexually inactive, poor self-rated general health and severe self-rated body pain (all P <0.05) among others. Conclusion The majority of the elderly were able to perform BADL independently and to a lesser extent IADL; the overall self-rated general health was found to be good. Further research in the field of geriatrics is warranted to plan better, and design public health policies and provide efficient care to elderly patients.

Background Dementia is affecting both the person with the disease and the family members. It is associated with nursing home admission, and a reduced ability to perform personal activities of daily living (P-ADL). The aim of this study was to examine the association between the severity of dementia and P-ADL function, and to study if additional factors such as neuropsychiatric symptoms, type of nursing home unit, and use of medication were associated with P-ADL function. Methods A total of 582 nursing home residents with dementia, included at admission to the nursing home, were followed with biannual assessments for 36 months. P-ADL was assessed using the Physical Self-Maintenance scale, and severity of dementia was measured with the Clinical Dementia Rating scale. In addition, neuropsychiatric symptoms, general physical health, and use of medications were assessed at the same time points. Demographic information was collected at baseline. Linear mixed models were estimated. Results There was a significant (p < 0.05) non-linear decline in P-ADL function over time in analysis not adjusting for any characteristics. More severe dementia at baseline and at the follow-up assessments was associated with lower P-ADL function (p < 0.001), with the association being stable over time. A higher level of neuropsychiatric symptoms, not using anti-dementia medication, being in a regular care unit as compared to a special care unit and having poor/fair general physical health as compared to good/excellent, were associated with a lower P-ADL function. Conclusion The association between more severe dementia and lower P-ADL function was stable over a 36-month follow-up period of nursing home residents with dementia. Health care planners and clinicians should be aware of this when planning for and treating nursing home residents.

Objetivo: Verificar a dependência funcional para a realização das atividades básicas, instrumentais e avançadas da vida diária entre idosos e seus fatores associados. Método: Estudo transversal e analítico, com 1.611 idosos da comunidade de uma Macrorregião de Minas Gerais. Na coleta dos dados utilizaram-se: Miniexame do Estado Mental; Questionário estruturado elaborado pelo Grupo de Pesquisa em Saúde Coletiva; versão brasileira Short Physical Performance Battery; Índice de Katz; Escalas de Lawton & Brody e Atividades Avançadas da Vida Diária e fenótipo de fragilidade. Procederam-se as análises descritiva e regressão logística múltipla (p<0,05). Resultados: 6,5% dos idosos eram dependentes para as atividades básicas; 70,7% dependentes parcial/total para as atividades instrumentais e 25,1% tinham menor participação nas atividades avançadas. A dependência para as atividades básicas e instrumentais da vida diária associou-se à faixa etária de 80 anos ou mais (p= 0,001; p<0,001); presença de 5 ou mais morbidades (p= 0,023; p= 0,017); incapacidade/baixo desempenho físico (p<0,001; p<0,001) e pré-fragilidade/fragilidade (p= 0,015; p<0,001), respectivamente. A dependência para as atividades instrumentais também foi associada à ausência de escolaridade (p<0,001). Já a menor participação nas atividades avançadas associou-se à ausência de escolaridade (p<0,001); renda individual mensal ≤ 1 salário-mínimo (p<0,001); incapacidade/baixo desempenho físico (p<0,001) e pré- fragilidade/fragilidade (p<0,001). Conclusão: Os idosos apresentaram maior percentual de dependência para realização das atividades instrumentais. A saúde física está relacionada à dependência funcional do idoso, independente da complexidade das atividades cotidianas; e deve ser considerada no planejamento de intervenções direcionadas à prevenção e/ou reabilitação do declínio funcional nesse grupo etário.
