INTRODUCTION: In some areas of South Africa, people with impaired mobility are known to have difficulty accessing wheelchairs. This study aimed to critically appraise public healthcare policies addressing service delivery in terms of assistive devices in general, and more specifically wheelchairs within primary health care METHOD: South African public healthcare policies that address assistive device service delivery were identified and thematically analysed using retrospective deductive document analysis. The themes used correspond to the WHO guidelines for the provision of wheelchairs within the context of the guiding principles of primary health care i.e., accessibly, affordability, acceptability, appropriateness, and availability. An analysis was undertaken on how the provision of assistive devices under these principles was addressed RESULTS: Three policies were identified addressing assistive device service delivery i.e. The National Rehabilitation Policy, The Policy Framework and Strategy for Disability and Rehabilitation Services in South Africa 2015 - 2020, and the National Health Insurance Policy. The National Rehabilitation Policy addressed the majority of the WHO guiding principles on wheelchair provision. Guidelines for non-discriminatory and evidence-based service delivery and the availability of assistive devices were identified in all the documents. Scant attention was paid to service delivery of assistive devices for people residing in urban or peri-urban areas. There was a paucity of guidance on service delivery to people with impaired mobility who were unable to access health services, those unaware of the services available and those who were not aware of the benefits of an assistive device CONCLUSION: The policies, viewed together, addressed all the WHO guiding principles, although some gaps were observed in each policy. A proposal emanating from this review was that an update of the National Rehabilitation Policy would address these deficiencies Keywords: assistive devices, health policy, impaired mobility, disability, rehabilitation

Health resources play an essential role in realizing the unattainable Sustainable Development Goals targets in lowering the maternal mortality ratio (MMR) globally. The study aimed to analyze trends and determinants of MMR based on the number of healthcare resources in Indonesia. This study is an explanatory study using repeated cross-sectional data from Central Java 2016-2020. Data were analyzed using bivariate analysis with scatter plots, statistical analysis with Pearson correlation tests, and a linear regression model using Tableau software. Results showed that the MMR decreased in 2016-2019. However, it increased in 2020. The ratio of obstetrics-gynecology specialists and nutritional personnel had an impact on the MMR in 2016. In 2017, the ratio of general practitioners affected the MMR, whereas, in 2019 and 2020, the ratio of nutritional workers had an effect. Our study concluded that the trend of MMR in Indonesia was affected by the number of healthcare personnel resources. It is recommended that the Indonesian government should fulfill the quantity and equalization of the distribution of healthcare personnel resources particularly the obstetrics-gynecology specialists, general practitioners, and nutritional workers so the MMR could be decreased.

Background Medication labels are often the only information available to patients after obtaining medication from a healthcare practitioner. Pictograms are graphic symbols that have shown to increase understanding of medicine use instructions. Aim To compare the accuracy of the interpretation of medicine use instructions from two different oral rehydration (OR) dry-mixture sachet labels – the control ‘routine textonly’ label and an experimental label with ‘text-and-pictograms’. Setting Participants were recruited from waiting rooms in public primary health care (PHC) facilities in Cape Town. Method Each participant was required to answer six questions about OR preparation. Response accuracy was determined by comparing the participant’s answer to the actual information written on the relevant label. Afterwards, participants could offer their opinion about the label and ways to improve their understanding. Results Of the 132 participants who were recruited, 67 were allocated to the experimental group and 65 to the control group. Only the significant difference between the experimental and control groups for the six questions regarding the label, was recorded for the answer that could be read from a single pictogram ( p = 0.00) on the experimental group’s label. When asked about this question, more control participants (15/65) found the dosing instruction difficult to understand when compared to the experimental group (1/67). A third of the control participants (22/65) indicated that they could not see or locate instructions on the label. Conclusion Text and pictograms on written medicine labels may be an effective tool to aid understanding of medicine use instructions amongst patients attending PHC facilities.
