Atopic dermatitis (AD) is a chronic relapsing inflammatory skin disease [1]. It affects 10%-20% of children, the most prevalent form being chronic disease, and 1%-3% of adults, particularly in the most developed Western societies [1][2][3]. Fifty percent of cases resolve during adolescence, with up to 20% of cases persisting into adulthood and a prevalence of 1%-3% in patients aged more than 60 years [4].Available evidence on the prevalence of severe AD is uncertain and scarce at the population level. No studies on the prevalence of AD have focused on Spain. The main goal of this study was to describe the prevalence of severe AD in routine clinical practice at different locations in Spain. As a secondary goal, associated comorbidities and therapeutic approaches were described.A cross-sectional (prevalence) study was conducted based on a review of medical registries (digital databases, dissociated data). The study population was obtained from health care providers' registries, which were unified in the dissociated database of the RedISS (Red de Investigación en Servicios Sanitarios) Foundation. Data came from the OMIap digital clinical history and other complementary databases. Registries from patients in Asturias, Catalonia, and the Balearic Islands were obtained. Patients who required healthcare services during 2015-2016 were included. The inclusion criteria were age ≥18 years, registered diagnosis of severe AD at least 12 months before the study, inclusion in the prescription program (registered daily dose, timing and duration of each treatment; ≥2 prescriptions during the follow-up period), and ability to guarantee regular follow-up (≥2 health registries in the system; at least 1 visit to the dermatology service). The exclusion criteria were change in health center or move outside the study areas, permanent hospitalization, and history of seborrheic dermatitis, contact dermatitis, and/or dyshidrotic eczema.Registries of patients with AD were obtained from the International Classification of Primary Care in the European Union [5] (codes S87, S88) and/or the International Classification of Disease (Ninth Edition), Clinical Modification (ICD-9-CM; 691.8, 692.9, 706.8), which include AD, allergic dermatitis, allergic eczema, atopic eczema, atopic neurodermatitis, and xerosis. AD was diagnosed by the physician (reference dermatologist) according to the criteria of Hanifin and Rajka [6] (AD present for over 1 year). AD was considered severe when the patient received an immunosuppressant (cyclosporine, methotrexate, azathioprine, mycophenolate mofetil) or a biologic (omalizumab, rituximab) during followup or had been hospitalized because of AD. Furthermore, administration of systemic corticosteroids and other treatments (immunoglobulins, interferon g, hydroxychloroquine, alefacept) received by the patient was registered. Introduction. Atopic dermatitis is a frequent chronic skin disease in children. The major clinical manifestations include itching and dryness of the skin. The pathomechanism of skin changes results from an interaction of genetic and environmental factors as well as impairments of skin barrier function and immune response. Despite chronic treatment the disease is characterized by exacerbation and remission periods and lowers the quality of life of patients and their families. Objective. To evaluate treatment satisfaction in children with atopic dermatitis, identify components of medical care which contribute to treatment satisfaction, and evaluate the relationship between satisfaction and adherence to a doctor's recommendations. Material and methods. One hundred and nineteen children (6 months to 12 years old, mean age 4.9 years) with atopic dermatitis were enrolled in the study. The doctor performed physical examinations and history taking and filled in questionnaires evaluating the course and exacerbation of the disease, the type of administered therapy and diagnostics. The patients' parents completed two questionnaires: a questionnaire assessing satisfaction with the therapy (the type of recommended therapy, adherence to recommendations, contact with the doctor, obtained information, degree of psychological support, role of parents in taking decisions regarding the therapy) and a quality of life questionnaire. Results. The authors observed that 56% of parents were dissatisfied with the administered treatment, and 40% failed to adhere to at least one therapeutic recommendation. Parents of children with mild atopic dermatitis significantly more often stop using emollients. It was also observed that lack of treatment satisfaction in children with severe atopic dermatitis whose parents are insufficiently educated contributes to decreased adherence. The authors identified independent factors of lack of treatment satisfaction: failure to obtain enough psychological support (OR = 20.00), failure to obtain reliable information on the disease (OR = 5.91), failure to actively get engaged in the treatment procedure (OR = 5.31), dissatisfaction with the contact with the doctor (OR = 4.9), dissatisfaction with the diagnostics (OR = 4.93) and severe course of the disease (OR = 2.82). Conclusions. Elimination of risk factors of dissatisfaction with the treatment might potentially improve the adherence to chronic treatment in atopic dermatitis. atopic dermatitis, satisfaction with treatment. Psoriasis is a chronic, recurrent immune-mediated skin disease with a 2-3% prevalence in the Western population, which severely affects patients' quality of life and poses a considerable socioeconomic challenge. The majority of individuals have psoriasis in limited areas and topical products are the mainstay of therapy according to existing guidelines. It is known that medication adherence rates are lower for topical treatment than for systemic treatment.Poor medication adherence is a major multidimensional problem for patients with chronic disorders as it is associated with unfavourable treatment outcomes, increased risk for development of concomitant diseases and inefficient use of health resources. Despite four decades of adherence research and the large number of studies that have identified the importance of medication non-adherence, there are relatively few studies reporting and designing effective strategies to improve adherence. The aim of this article was to report and describe non-adherence in the topical treatment of psoriasis, the factors that might contribute to this phenomenon, and which interventions have so far been developed for the management of chronic conditions. This article proposes that given that the barriers to medication adherence are complex and varied, solutions to improve adherence and thus clinical outcomes must be multifaceted and must also provide the possibility to be tailored according to each patient's individual needs.Such an individualized and comprehensive adherence-enhancing intervention would probably enable the successful long-term management of this disabling chronic condition.
