Background: Numerous studies have reported on pain in dementia. It has been hypothesized that pain perception differs between dementia subtypes, and therefore, the prevalence of pain differs between dementia subtypes. However, there remains a paucity of evidence on the differences in the prevalence of pain in different dementia subtypes. This review aimed to determine the prevalence of pain for the major dementia subtypes: Alzheimer's disease (AD), vascular dementia (VaD), frontotemporal dementia (FTD) and dementia with Lewy bodies (DLB). Summary: We found 10 studies that met our inclusion criteria. Most of these studies reported on AD; studies reporting the prevalence of pain in people with DLB were scarce, and for FTD, we found no studies. The sample-weighted prevalence of pain could only be calculated for AD, VaD and mixed dementia: AD 45.8% (95% confidence interval, CI: 33.4-58.5%), VaD 56.2% (95% CI: 47.7-64.4%) and mixed dementia 53.9% (95% CI: 37.4-70.1%). Key Messages: Studies investigating the prevalence of pain in dementia subtypes were scarce; however, we found a high prevalence of pain in dementia without significant differences between the dementia subtypes. More studies are required to draw firm conclusions on the differences in the prevalence of pain between dementia subtypes.

RESUMENFundamentos: Es conocida la asociación inversa que existe entre la calidad de vida relacionada con la salud (CVRS) y el consumo de consultas. Sin embargo, el sexo es considerado habitualmente como variable de confusión, y son escasos los estudios que han analizado explícitamente las diferencias de sexo en esta cuestión. El objetivo fue conocer la influencia de la CVRS sobre el uso de consultas de Atención Primaria en las personas >65 años, en un análisis diferenciado por sexo.Métodos: La selección de participantes se realizó a lo largo de 2013, 191 mujeres y 155 varones de ≥65 años fueron estudiados mediante entrevista y análisis de historias clínicas. Se utilizó el Cuestionario EuroQol-5D y fueron analizadas variables demográficas, clínicas y de apoyo social. Se elaboraron 2 modelos de regresión múltiple, uno dirigido a las mujeres y otro a los varones.Resultados: La CVRS se correlacionó de forma negativa con el consumo de consultas (Rho-Spearman=-0,22; p=0,0001), con una OR cruda de 1,85 (IC95%: 1,2-2,9). La asociación se mantuvo significativa al ajustar tanto por variables demográficas [OR=1,99 (IC95%: 1,2-3,2)], como por variables clínicas [OR=1,79 (IC95%: 1,1-2,9)] o de apoyo social [OR=1,83 (IC95%: 1,1-2,9)]. En el análisis de regresión, los valores del coeficiente estandarizado (β) asociado a la CVRS fueron 0,22 (IC95%: -36,7--6,9) en las mujeres, y 0,03 (-15,6-23,1) en los varones.Conclusiones: En las mujeres de ≥65 años la CVRS es la variable con mayor capacidad explicativa de consumo de consultas, tras ajustar por variables demográficas, clínicas y de apoyo social. Objective. Cost-of-illness analysis is the main method of providing an overall vision of the economic impact of a disease. Such studies have been used to set priorities for health care policies and inform resource allocation. The aim of this study was to determine the economic burden from a societal perspective and health-related quality of life (HRQOL) of patients with systemic sclerosis (SSc; scleroderma) in Spain. Methods. We conducted a cross-sectional, retrospective study of 147 patients with SSc. Data on demographic characteristics, health resource utilization, informal care, labor productivity losses, and HRQOL were collected from the questionnaires completed by patients or their caregivers. HRQOL was measured with the EuroQol 5-domain (EQ-5D) questionnaire. Results. The average annual cost for an SSc patient was €21,042; the reference year for unit prices was 2011. Direct health care costs amounted to €8,235, direct non-health care costs were €5,503, and loss of labor productivity was €7,303. The largest expenditures were early retirement, informal care, and medication. There are sharp differences in the estimated cost according to the Barthel Index score observed. The mean EQ-5D score for SSc patients was 0.68 and the mean EQ-5D visual analog scale score was 64. Conclusion. The main strengths of this study lie in our bottom-up approach to costing and in the evaluation of SSc patients from a broad perspective (societal costs). This type of analysis is very scarce in international literature for rare diseases in comparison with other illnesses. We conclude that SSc patients incur considerable societal costs and experience substantial deterioration in HRQOL.
