Health laboratories in the Tanga region of Tanzania: the quality of diagnostic services for malaria and other communicable diseases

Abstract: Although critical for good case management and the monitoring of health interventions, the health-laboratory services in sub-Saharan Africa are grossly compromised by poor infrastructures and a lack of trained personnel, essential reagents and other supplies. The availability and quality of diagnostic services in 37 health laboratories in three districts of the Tanga region of Tanzania have recently been assessed. The results of the survey, which involved interviews with health workers, observations and a docu… Show more

“…Malaria diagnosis in Africa is still largely based on clinical signs and symptoms, with the role of laboratories significantly diminished or nonexistent. 5,6 A consequence of the prolonged neglect of laboratories in malaria case management is that, even when quality laboratory services are available, clinicians too frequently ignore negative results. [7][8][9][10][11][12] The reduction in clinical malaria in several countries [13][14][15][16] and the use of expensive artemisinin-based combination therapeutic (ACT) antimalarial drugs call for case management practices that emphasize parasitologic confirmation, because continued use of ACTs in non-malaria cases represents a waste of resources and eventually, is unsustainable.…”

“…In many such settings, the need for alternative technologies is also pressing, because routine culture and susceptibility testing are not provided, even to support diagnosis of lifethreatening infections like pneumonia and meningitis (fi gure 4). 132 Immunoassays provided a faster option, but the potential for changing the landscape of diagnostic testing became clear with the development of the PCR in the 1980s. 133 Since then, we have seen a technological revolution with the development of many complex, highly specifi c molecular diagnostic assays.…”

“…Identification of errors and rectification strategies in laboratory medicine has received a great deal of attention [5]. Although medical laboratory service provision is a well developed science, observations indicate that many laboratories turn out hazardously inferior work resulting in inaccurate determinations and failure to identify the causative agents in samples [2]. Globally, international differences exist in the guidelines on good quality laboratory services.…”

“…Many medical hospital, public health, and academic laboratories in both the public and private sector contribute through their diagnostic activities to health care and public health improvement. In addition, animal health, food safety, and environmental health laboratory services contribute to health care and public health security [1,2].…”
