Aim:To compare medication prescriptions for patients with knee osteoarthritis (KOA) in the real world in Korea and Japan.Methods: This retrospective and descriptive population-based study was conducted using claims data provided by Health Insurance Review and Assessment in Korea and JMDC Inc in Japan. We defined individuals as KOA patients if they had an International Classification of Diseases 10 (ICD10) code for gonarthrosis (M17) and were ≥50 years old in 2012. Korean and Japanese patients were matched for age and sex using frequency matching. Patients were observed for 1 year from the first month of the ICD10 code M17 in 2012. We described baseline characteristics including prevalence of comorbidities, and use of medication for KOA during the observational period. Results:The median age was 59 and the percentage of women was 61.4 in both countries (N = 1 133 138 in Korea, N = 10 498 in Japan). The prevalence of nonsteroidal anti-inflammatory drug (NSAID) usage in Japan (74.7%) was significantly higher than that in Korea (59.0%). Analgesics such as acetaminophen and symptomatic slow-acting drugs for OA (SYSADOA) were significantly more frequently used in Korea (25.8%) than in Japan (9.4%). Intra-articular injection (IAI) of corticosteroids or hyaluronic acid was performed more frequently in Japanese patients (57.3%) than Korean patients (30.5%). Conclusions:Medication patterns for KOA in Korea and Japan are described for the first time. Use of NSAIDs and IAI were more common in Japan, while other pain killers and SYSADOA were more commonly prescribed in Korea. 1Osteoarthritis (OA) is a debilitating and prevalent chronic disease, but there are no 2 approved disease modifying OA drugs (DMOADs), only pharmaceuticals for pain management. 3 OA progression, particularly for post-traumatic osteoarthritis (PTOA), is associated with 4 inflammation and enzymatic degradation of the extracellular matrix. In particular, Matrix 5Metalloproteinase 13 (MMP13) breaks down collagen type 2 (CII), a key structural component of 6 cartilage extracellular matrix, and consequently, matrix degradation fragments perpetuate 7 inflammation and a degenerative cycle that leads to progressive joint pathology. Here, we tested 8 targeted delivery of endosome-escaping, MMP13 RNA interference (RNAi) nanoparticles (NPs) 9 as a DMOAD. The new targeting approach pursued here deviates from the convention of targeting 10 specific cell types (e.g., through cell surface receptors) and instead leverages a monoclonal 11 antibody (mAbCII) that targets extracellular CII that becomes uniquely accessible at early OA 12 focal defects. Targeted mAbCII-siNPs create an in situ NP depot for retention and potent activity 13 within OA joints. The mAbCII-siNPs loaded with MMP13 siRNA (mAbCII-siNP/siMMP13) 14 potently suppressed MMP13 expression (95% silencing) in TNFa-stimulated chondrocytes in 15 vitro, and the targeted mAbCII-siNPs had higher binding to trypsin-damaged porcine cartilage 16 than untargeted control NPs. In an acute mechanical injury mouse model of PTOA, mAbCII-17 siNP/siMMP13 achieved 80% reduction in MMP13 expression (p = 0.00231), whereas a non-18 targeted control achieved only 55% silencing. In a more severe, PTOA model, weekly mAbCII-19 siNP/siMMP13 long-term treatment provided significant protection of cartilage integrity (0.45+/-20 .3 vs 1.6+/-.5 on the OARSI scale; p=0.0166), and overall joint structure (1.3+/-.6 vs 2.8+/-.2 on 21 the Degenerative Joint Disease scale; p<0.05). Confidence and Attitudes Toward Osteoarthritis Care Among the Current and Emerging Health Workforce: A Multinational Interprofessional Study

Objective. To measure confidence and attitudes of the current and emerging interprofessional workforce concerning osteoarthritis (OA) care.Methods. Study design is a multinational (Australia, New Zealand, Canada) cross-sectional survey of clinicians (general practitioners [GPs], GP registrars, primary care nurses, and physiotherapists) and final-year medical and physiotherapy students. GPs and GP registrars were only sampled in Australia/New Zealand and Australia, respectively. The study outcomes are as follows: confidence in OA knowledge and skills (customized instrument), biomedical attitudes to care (Pain Attitudes Beliefs Scale [PABS]), attitudes toward high-and low-value care (customized items), attitudes toward exercise/physical activity (free-text responses).Results. A total of 1886 clinicians and 1161 students responded. Although a number of interprofessional differences were identified, confidence in OA knowledge and skills was consistently greatest among physiotherapists and lowest among nurses (eg, the mean difference [95% confidence interval (CI)] for physiotherapist-nurse analyses were 9.3 [7.7-10.9] for knowledge [scale: 11-55] and 14.6 [12.3-17.0] for skills [scale: 16-80]). Similarly, biomedical attitudes were stronger in nurses compared with physiotherapists (6.9 [5.3-8.4]; scale 10-60) and in medical students compared with physiotherapy students (2.0 [1.3-2.7]). Some clinicians and students agreed that people with OA will ultimately require total joint replacement (7%-19% and 19%-22%, respectively), that arthroscopy is an appropriate intervention for knee OA (18%-36% and 35%-44%), and that magnetic resonance imaging is informative for diagnosis and clinical management of hip/knee OA (8%-61% and 21%-52%). Most agreed (90%-98% and 92%-97%) that exercise is indicated and strongly supported by qualitative data.Conclusion. Workforce capacity building that de-emphasizes biomedical management and promotes high-value first-line care options is needed. Knowledge and skills among physiotherapists support leadership roles in OA care for this discipline.
