Healing Sacral Fracture Masquerading as Metastatic Bone Disease on a 68Ga-PSMA PET/CT

Abstract: Prostate-specific membrane antigen (PSMA) is a cell surface glycoprotein, which is frequently overexpressed on prostate cancer cells. A Ga-PSMA PET/CT can be used for early detection of lymph node or bone metastases after radical prostatectomy when there is biochemical recurrence. This report describes PSMA uptake in a healing fracture masquerading as metastatic bone disease in a patient with a history of prostate adenocarcinoma. Clinicians reporting Ga-PSMA PET/CT should be aware of this potential important p… Show more

“…Furthermore, Gykiere et al presented a case of PSMA uptake in a healing sacral bone fracture mimicking bone metastasis. MRI revealed no evidence of an underlying pathologic fracture, but no biopsy was conducted to verify or rule out pathologic fracture (Gykiere et al, 2016).…”

“…It can often be difficult to determine if the rib lesion is a benign lesion or represent metastases from prostate cancer. Differential diagnoses include healed fractures [9][10][11], sarcoidosis [12,13], fibrous dysplasia [14,15] and Paget's disease [16,17], and literature on this topic is limited to a small number of case reports. Given the lack of data in the current literature, we conducted a retrospective review investigating cases of solitary rib lesions with PSMA uptake on pre-treatment 68 Ga-PSMA PET/CT.…”

“…However, a wide range of malignancies other than prostate cancer have also been reported to express PSMA as part of tumor neovasculature [ 7 – 9 ] with 68Ga-PSMA avidity described in cases of breast cancer [ 10 ], renal cell carcinoma [ 11 ], glioblastoma multiforme [ 12 ], hepatocellular carcinoma [ 13 ], differentiated thyroid cancer [ 14 ], colorectal carcinoma [ 15 ], non-small cell lung cancer [ 16 ] and follicular lymphoma [ 17 ]. PSMA uptake has been also reported in a large variety of benign lesions such as retroperitoneal schwannoma [ 18 ], desmoid tumor [ 19 ], Paget’s disease of bone [ 20 ], sarcoidosis [ 21 ], sub-acute stroke [ 22 ] and bone fractures [ 23 , 24 ]. Uptake in benign processes as well as in the celiac ganglia can mimic a lymph node metastasis [ 24 , 25 ] and can therefore be pitfalls in clinical practice.…”
