Healing Mechanism and Clinical Application of Autogenous Tooth Bone Graft Material

The purpose of this study was to evaluate the DDM as a carrier of recombinant human BMP-2 (rhBMP-2). In the study 1, the rhBMP-2 was fixed to inorganic bovine bones, tricalcium phosphate (TCP), DDM powder, and DDM chip. Over a period of 36 days, the amount of rhBMP-2 released was analyzed. In the study 2, the expression of protein markers by rhBMP-2 combined with DDM was evaluated in mice at 1, 2 and 4 weeks. In the control group (n=20 mice), 20 μg grafts of TCP+rhBMP-2 (concentration=0.05 mg/ml) were applied, while in the experimental group (n=20 mice), 20 μg grafts of DDM powder+rhBMP-2 (concentration=0.05 mg/ml) were applied. In the study 1, the slow release of rhBMP-2 was exhibited by all four graft types throughout the 36-day, a statistically significant large amount of rhBMP-2 was released from the DDM powder compared to the other materials (p<0.05). In the study 2, the expression of osteonectin was found to be limited to adjacent areas of TCP in the control group, however, it was observed in the DDM in the experimental group. The experimental group exhibited significant mature bone formation in the 4th week, but not in the 1st week of the experiment. Within limited research in this study DDM powder appears to have great potential as an effective scaffold for rhBMP-2. In the DDM powder+rhBMP-2, new bone formation was evidently shown and the amount of released rhBMP-2 was the highest. It was proposed that DDM powder might be an effective carrier of rhBMP-2.

Demineralized Dentin Matrix as a Carrier of Recombinant Human Bone Morphogenetic Proteins: <i>in Vivo</i> Study

This study aimed to evaluate the effi cacy of rabbit demineralized dentin matrix (DDM) as a recombinant human bone morphogenetic protein-2 (rhBMP-2) carrier using the subcutaneous tissues of mice and rabbit calvarial critical-sized defects. DDM of rabbit, combined with rhBMP-2 (DDM/rhBMP-2) was transplanted into the subcutaneous tissues of 6 mice and 6 rabbit calvarial critical-sized defects (DDM = 0.03 g, control; DDM/rhBMP-2 = 0.03 g of DDM, 0.2 mg/ml, 5.0 μg of rhBMP-2, experimental). Both DDM and DDM/rhBMP-2 was transplanted into the left and right subcutaneous tissues of mice symmetrically. For rabbits, 4 round critical-sized defects (8 mm diameter) were formed on the exposed skull. DDM was transplanted into the 2 defects on the left sides (n = 12) and DDM/rhBMP-2 into the right sides (n = 12). Two animals among 6 mice and 6 rabbits were sacrifi ced respectively at the 1, 2, and 4 experimental weeks for the histological and histomorphometrical evaluations with hematoxylin and eosin staining. Tissues from rabbits were imaged via micro-computed tomography (μCT). DDM/rhBMP-2 in mice induced new bone formation at 2 weeks and maturation with bone marrow at 4 weeks. DDM/rhBMP-2 in rabbit calvarium induced new bone formation remarkably at 4 weeks 21.77-47.99% compared to the DDM. These observations suggest that DDM could be considered a potential carrier of rhBMP-2. The rhBMP-2 loaded on DDM enhanced bone formation.

The purpose of this study is to evaluate an ability of a moldable autogenous tooth bone graft material (M-AutoBT), which is made of demineralized dentin matrix (DDM) and hydroxypropylmethyl cellulose (HPMC) to repair sinus-related defects in the implant dentistry. Sinus-related alveolar defects were classified into three types depending on the remnant of socket wall as follows; Class I: Only alveolar bone repair is required even if sinus cavity is involved (remained socket wall > 5mm). Class II: Both alveolar bone and sinus repair are required (3mm < remained socket wall < 5mm). Class III: Only sinus repair is required without any alveolar bone (remained socket wall < 3mm). Sinus cavity should always be involved during the implant placement regardless of defect types. The histological examination of three case reports, which represented class I, II and III defect respectively, showed osteoconductive and osteoinductive functions of M-AutoBT that is similar to the effect of the conventional AutoBT. Therefore, HPMC might be the suitable base material for AutoBT.
