Мигрень -хроническое, широко распростра-ненное заболевание, значительно снижающее каче-ство жизни пациентов и требующее больших эконо-мических затрат здравоохранения. Так, распростра-ненность этой первичной цефалгии среди взрослого населения, по данным ВОЗ, составляет свыше 10%.Центральным звеном клинической картины мигре-ни является интенсивная пульсирующая головная боль, чаще по типу гемикрании, усиливающаяся при физических нагрузках, нередко сопровождаю-щаяся тошнотой и рвотой. Однако это заболевание отличается значительной клинической гетероген- Цель -изучить распространенность гастроэзофагеальной рефлюксной болезни и оценить влияние этого заболевания на качество жизни пациентов с мигренью. Материал и методы. Обследованы 104 больных мигренью в возрасте от 18 до 60 лет. Изучалось наличие и интенсивность изжоги в иктальный и интериктальный периоды мигрени. Всем больным была проведена эзофагогастродуоденоскопия. Оценка качества жизни, связанного со здоровьем, проводилась с помощью опросника MOS SF-36. Результаты. Изжога вне мигренозных пароксизмов наблюдалась в 48 (46,2%) случаев. Гастроэзофагеальная рефлюксная болезнь верифицирована у 57 (54,8%) больных. Пациенты с сочетанной патологией были достоверно старше и имели более высокий индекс массы тела. В этой группе наблюдалась тенденция к увеличению количества мигренозных приступов, интенсивности головной боли, степени дезадаптации. Корреляционный анализ выявил зависимость между внеприступной изжогой и интенсивностью изжоги, беспокоящей пациентов во время болевой фазы мигрени, а также частотой мигренозных приступов. Показатели всех шкал качества жизни (за исключением шкалы телесной боли) были ниже в группе больных с сочетанной патологией. Заключение. Таким образом, изжога и гастроэзофагеальная рефлюксная болезнь широко распространены среди больных мигренью и имеют тенденцию к нарастанию частоты в группе больных хронической формой цефалгии. Качество жизни пациентов с сочетанной патологией значительно ниже в сравнении с больными, страдающими только мигренью. Aim. The objective of the present work was to study the prevalence of gastroesophageal reflux disease and to evaluate the influence of this condition on the quality of life of the patients suffering from migraine. Material and methods. A total of 104 patients suffering from migraine were available for the examination. The age of the patients varied from 18 to 60 years. Special emphasis was placed on the detection of heartburn and evaluation of its severity during the ictal and interictal periods of migraine. All the patients underwent oesophagoduodenoscopy. The health-related quality of life was estimated with the use of the MOS SF-36 questionnaire. Results. Heartburn in the absence of migrainous attacks was shown to occur in 48 (46.%) patients while gastroesophageal reflux disease was diagnosed in 57 (54.8%) of them.
