Head-and-neck paragangliomas: An overview of 54 cases operated at a tertiary care center

Abstract: Background:Head-and-neck paragangliomas (HNP's) are rare autonomic neoplasms associated with high morbidity and mortality. We aimed to study epidemiology, clinicopathological correlation, and management of HNP to assist clinicians in advocating the most appropriate therapy.Materials and Methods:Epidemiological parameters, including age and sex distribution, clinical presentation, tumor classification, familial predisposition, multicentricity, and treatment modalities adopted, were analyzed in this retrospectiv… Show more

“…It is well known that HNPGLs occur more frequently in women than men ( Erickson et al 2001 , Smith et al 2017 , Rijken et al 2019 , Singh et al 2019 , Rana et al 2021 ) and this was also reflected in our cohort. We systematically analyzed clinical features of HNPGLs and demonstrated that tumors of jugular and tympanic origin are more common in women (88%) than men.…”

“…Surgical resection represents the mainstay of treatment for HNP and is associated with the highest rates of local control. 19 , 20 However, due to the morbidity associated with surgery, recent studies have suggested that primary radiotherapy may provide similar local tumor control with reduced morbidity. 21 - 23 Others have suggested that conservative management with serial imaging and close follow-up may be preferred for patients with advanced age, multiple comorbidities, small asymptomatic tumors, or cases in which surgical resection carries high morbidity.…”

“…Carotid body tumors are the most frequent head and neck paragangliomas, although they are rare when all head and neck tumors are considered. 3,4 Their definitive treatment is surgery in expense of neurovascular complications. 5 The efficacy of angiography and embolization before CBT surgery was first reported by Schick et al 16 The aim of embolization is to control arterial supply of the tumor and by this way to decrease blood loss, to shorten the operation time, and to decrease overall surgical complications.…”

“…2 Head and neck paragangliomas constitutes less than 0.5% of all head and neck tumors and approximately 0.03% of all human tumors. 3 Head and neck paragangliomas are named according to their site of origin (eg, carotid body, jugular, vagal, tympanic, etc), and carotid body tumor (CBT) is the most frequent head and neck paraganglioma with a rate of 60% to 78%. 4 Although being mostly benign, malignancy rate of 6% to 10% is reported for CBT.…”
