Head and neck cutaneous metastasis of testicular choriocarcinoma

Abstract: Testicular choriocarcinoma (CC) is a malignant germ cell tumour which most frequently presents with disseminated metastasis, often involving the lungs, brain and liver. Metastatic are characterised by extensive vascularity, often causing patients to present emergently with potentially life-threatening haemorrhagic complications. We report a patient with disseminated testicular CC, presenting with haemorrhage from a dermal metastatic focus involving the lower lip and mentum, requiring surgical intervention. Thi… Show more

“…Due to its pronounced invasiveness into blood vessels and tissues, extensive hemorrhage, necrosis, and lymphovascular invasion are common findings ( 67 ). Choriocarcinoma primarily metastasizes hematogenously, resulting in early and extensive dissemination to sites such as the lungs, liver, skin, retroperitoneal lymph nodes, gastrointestinal tract, and central nervous system ( 37 , 68 71 ). In this case, the diagnosis of choriocarcinoma was established at an advanced stage, with multiple metastases already present throughout the body.…”

“…Hematogenic spread is characterized by predominant involvement of the lungs, brain, and occasionally liver and retroperitoneal lymph nodes [ 8 ]. There were individual cases of metastatic lesions of the skin [ 9 ] and pancreas [ 10 ]. Metastasis of CC into the stomach is a very rare phenomenon.…”

“…15 Due to its high-vasculature, choriocarcinoma cutaneous metastasis usually manifest as hemorrhagic nodules, red papules resembling hemangiomas, or occasionally as asymptomatic subcutaneous nodules, 16 which can be solitary, as in our case, or multiple. Metastasis to unusual skin sites including nasal skin, 17 the lip, 5 or the little finger 18 are on record. Cutaneous metastasis to the back, similar to our case, has only been reported in another case of a 22 year old male patient, albeit, as multiple nodules, and the patient died 3 months later.…”

“… 4 Cutaneous metastatic choriocarcinoma is a rare encounter with a few cases reported in literature. 5 In this case, we present a patient with testicular choriocarcinoma that progressed to develop metastatic cutaneous lesion.…”
