HCV liver disease in renal transplantation: a clinical and histological study

Abstract: The prevalence of HCV infection is high in renal transplantation (RT) patients: 29% in our cohort of 399 RT recipients. The consequences of that infection on the liver have to be carefully assessed. Clinical chronic hepatitis was detected from ALT concentrations (> x 1.5 N) in only 26 patients (22%) with constant (15%) or fluctuating (85%) ALT elevation. Only three of 117 cases developed cirrhosis (3%). No liver cancer was noted. Liver biopsy was performed (mean interval = 60.2 months) in 62 patients with HCV … Show more

“…About 80% of HCV− patients and 65% of HCV+ patients are alive in that period 43,83–85 . It is thought that immunosuppressive drugs (e.g., azathioprine) and hepatotropic viruses (e.g., HBV, CMV, and HIV) may intensify hepatocellular damage 25,86 . The prevalence of liver diseases in HCV− recipients who received the kidney from an anti‐HCV+ donor is approximately 4.4 times higher than that of seronegative donors (55% vs. 16%) 80,87 .…”

“…There is also an increased susceptibility for sepsis in post-transplant HCV-positive patients [16, 17]. Several reports on the renal transplant and hepatitis positivity disagree with these findings [18, 19]. …”
