Has COVID-19 Modified the Weight of Known Systemic Inflammation Indexes and the New Ones (MCVL and IIC) in the Assessment as Predictive Factors of Complications and Mortality in Acute Pancreatitis?

Abstract: We aimed at evaluating the prognostic capacity of the inflammatory indices derived from routine complete blood cell counts in two groups of patients with acute pancreatitis from two different time periods, before and during the COVID-19 pandemic, when a high incidence of complications with surgical risk and mortality was found. Two new markers were introduced: the mean corpuscular volume to lymphocyte ratio (MCVL) and the cumulative inflammatory index (IIC), which were calculated at a baseline in the two group… Show more

“…During the global chaos of the first wave of COVID-19, the outcomes of acute abdominal pathology were negatively affected ( 6 ) and millions of elective surgical procedures were canceled ( 68 ). This move allowed the redistribution of resources and healthcare personnel towards the additional care needs of patients with COVID-19.…”

“…Radulescu et al [50] were the first to have highlighted new hematological indices calculated according to the increased values of the red cell distribution width (RDW), and the mean corpuscular volume (MCV), and they obtained the ratio between the mean corpuscular volume and lymphocytes (MCVL), and the cumulative inflammatory index (IIC) ((mean corpuscular volume × width of erythrocyte distribution × neutrophils)/(lymphocytes × 1000)). These new hematological indices better reflect the inflammation-induced changes to red and white cells in the peripheral blood following an inflammatory process.…”

“…The documentation by Wang et al25 first highlighted the possible harm inflicted on the pancreas by COVID-19. Subsequently, other case reports and retrospective investigations have emerged, indicating a plausible correlation between COVID-19 and pancreatitis, particularly AP.According to retrospective research conducted in Romania, it was shown that SARS-CoV-2 infection had a significant impact on the relative risk of mortality in patients with AP 26. In cohort research using the Hong Kong Hospital Authority and the UK Biobank database, Lin et al27 also documented pancreatitis as a persistent acute consequence of COVID-19 infection.…”
