Handling of iodothyronines by the liver and kidney in patients with chronic liver disease

Abstract: Possible arterio-venous gradients of T4, T3, rT3 and 3,3'-diiodothyronine (3,3'-T2) across the liver and the kidneys were measured in 9 patients with varying degrees of liver failure undergoing diagnostic catheterization. Plasma iodothyronine levels were measured in peripheral, hepatic and renal veins before and at 10-min intervals until 60 min after iv injection of 400 \ of TRH. In 2 patients estimated hepatic plasma flow and effective renal plasma flow were determined as well. In these 2 patients, no signifi… Show more

“…Plasma levels of rT3 were measured by established radioimmunoassay (RIA) procedures, as described previously. 14,15 Serum TSH, free T4, T4 and T3 levels were measured by Vitros Eci technology (Ortho-Clinical Diagnostics, Amersham, UK). The within-assay coefficient of variation (CV) was 3-7% and the between-assay CV 5-10%.…”

“…Production of plasma T3 and clearance of rT3 (Bauer et a/., 1987) occur predominantly in tissues with type I deiodinase activity, e.g. the liver, although a (probably minor) contribution of the kidneys is not excluded (Bauer et nl., 1987;Leonard, 1990).…”

“…This is, however, not the case, since serum rT3 is actually elevated in fasting and NTI (1,2). There are several reasons for this phenomenon: (i) The production of plasma rT3 primarily occurs outside the liver, while the liver is the major site for its clearance (65). The clearance of rT3 is reduced in NTI because of reduced entry of rT3 into the liver, followed by diminished deiodination.…”
