“…The EPIC review showed that liquid (even nonmedicated) soap and water effectively decontaminates hands, but that 70% alcohol or an alcohol-based antiseptic hand rub provides the most effective decontamination for a wide variety of organisms [20]. With hand rubbing the median percentage reduction in bacterial contamination was significantly higher than with hand washing (83% v 58%, P = 0.012), with a median difference in the percentage reduction of 26% (95% confidence interval 8% to 44%).…”

Nosocomial infections are a major source of morbidity and mortality in hospital settings. The most important defences against nosocomial transmission of viral, bacterial, and other infections are detailed and continuing education of staff and strict adherence to infection control policies. The issue is no longer whether hand hygiene is effective, but how to produce a sustained improvement in health workers' compliance.

Nosocomial infections remain a major side effect of medical care and are at least in part preventable. Evidence-based, valid infection control recommendations are needed based on multidisciplinary development with local adjustment and incorporation into every-day practice. The article reviews the special aspects which have to be considered for producing evidence based guidelines in the field of infection control. However, guidelines designed to prevent nosocomial infections may be ignored even in the face of persuasive scientific evidence. Insufficient compliance to hand hygiene is the most prominent example. Factors which impede the process of translating research findings into infection control practice have to be identified and suggestions how these barriers may be overcome should be provided.
