Haemorrhage following coronectomy of an impacted third molar associated with a bifid mandibular canal: a case report and review of the literature

Abstract: A bifid mandibular canal is a rare anatomical variant believed to result from failure in fusion of multiple embryonic canals. Although the anatomical variations have been well described, clinical sequelae from this anatomical variant have, to our knowledge, never been described before. We describe a case in which a partially erupted third molar closely associated with a bifid mandibular canal was treated with a coronectomy. The patient subsequently attended as an emergency with haemorrhage necessitating explor… Show more

“…After CBCT imaging, it was confirmed that the implants interfered with bifid canals and a significant reduction in pain was noted following the removal of the implants. A severe hemorrhagic complication related to the presence of a bifid canal was reported by Verea linares et al [ 43 ]. A partially erupted third molar, closely associated with a bifid canal, was treated with coronectomy.…”

“…The authors suggested performing a CBCT for appropriate surgical planning in a suspected bifid canal. Furthermore, they recommended complete removal of the tooth rather than coronectomy to allow direct access to the source of bleeding [ 43 ].…”

“…Therefore, these roots must be extracted immediately in order to limit an infectious complication as confirmed in the study by Pogrel MA [12] . Also, the lesion of the inferior alveolar nerve, the lingual nerve are represented as very rare complications that can occur [20] , [26] , [27] , [28] , without forgetting the risk of developing alveolitis or abscess postoperatively [30] .…”

“…Due to the neurovascular content, the BMC poses a risk for the patient as well as a challenge to the clinician. Several case reports have described intra-or postoperative complications related to damage of the BMC (Maqbool et al 2013;Aljunid et al 2016;Verea Linares et al 2016).…”
