SYNOPSIS Two patients are described who developed subarachnoid haemorrhage during pregnancy and the puerperium. In both patients there was clinical, haematological, histological, and renal angiographic evidence of the intravascular coagulation syndrome. No source of intracranial bleeding was demonstrated by bilateral carotid angiography in either patient. We suggest that the subarachnoid haemorrhage was a result of the intravascular coagulation syndrome in both patients.

Les auteurs rapportent 27 observations anatomo-cliniques, chez I’enfant, de «syndrome hémolytique et urémique» caractérisé par la survenue simultanée d’une anémie hémolytique, d’une atteinte rénale, d’une thrombopénie et de troubles neurologiques qui sont plus inconstants. L’atteinte rénale se manifeste soit par une anurie, soit par une néphropathie de type particulier: urémie très élevée, hématurie constante, protéinurie abondante s’accompagnant souvent d’une hypoprotidémie et parfois d’un syndrome néphrotique complet, perte rénale d’eau et d’electrolytes, de sodium et de potassium, hypertension artérielle précoce ou retardée. Les convulsions inaugurales sont fréquentes Certains cas sont complètement curables, surtout chez le nourrisson, mais des rechutes ultérieures et une hypertension tardive peuvent apparaître. Les lésions anatomopathologiques comportent trois types de lésions: micro-angiopathie thrombotique exclusivement rénale (10 cas); atteinte vasculaire disséminée de purpura thrombotique thrombocytopénique (2 cas); enfin, nécrose corticale (7 cas) dont un cas avec lésions vasculaires généralisées. Les auteurs envisagent les corrélations anatomo-cliniques et évolutives. Ils discutent des problèmes nosologiques posés par les diverses lésions anatomiques observées dans ce syndrome et sont partisans de I’unité histo-pathologique du syndrome.
