H2020 and Beyond: Skip Discrepancy between Theory and Practice of Personalized Medicine. A Position Paper by the Italian Society of Personalized Medicine

Abstract: Many unsolved practical issues, from technical and scientific to ethical, legal and economic topics, are slowing down the translation of Personalized Medicine principles into medical practice. The Italian Society of Personalized Medicine exposes here its point of view, based on the real-world practice of precision medicine carried-out in Italian healthcare structures.

“…It is a challenge not easy to win, essential for the optimal course of treatment success: mutual understanding and listening the other's point of view between doctors and patients is needed. Only then, personalized medicine can meet the challenge of being a positive development for future medicine [68].…”

“…The personalized medicine, emphasizing a clinical approach based on the uniqueness of each patient, has among its objectives to identify the preventive and/or therapeutic actions that best suits the needs of the single patient. It is known that a model of medicine aimed at the possibility of identifying the susceptibility of each person to possible diseases, measuring the risk, customizing the therapy according to the patient's genetic constitution and affecting the appropriateness of the drug administered, reduces the occurrence of adverse reactions [1]. Basic prerequisite is the inherent difference given by the genetic heritage that characterizes us, diversity that can affect our response to administered therapy, in particular to its effectiveness but also to the possible side effects According to the World Health Organization (WHO), an adverse drug reaction (ADR) is "a response to a drug which is noxious and unintended, and which occurs at doses normally used in man for the prophylaxis, diagnosis, or therapy of disease, or for the modification of physiological function."…”

“…Personalized medicine (PM), included in so called P4 medicine (Personalized, Predictive, Preventive and Participative medicine), besides a fifth P represented by emerging Precision medicine initiative, is an innovative approach to the patient, in a comprehensive effort integrated in the common frame of P5 medicine. As a result, rather than accepting the one-size-fits-all, the need to be tailored to fit the profile of each individual patient is an emerging practice [1]. In this way, new approaches are critical to adapt clinical treatment to the individual and to propose available alternatives taking into account the specific profile of the person.…”
