Background The aim of this study was to compare the effects of nalbuphine and sufentanil on the gastrointestinal (GI) tract after laparoscopic surgery for gynecological malignancies. Methods A total of 100 patients aged between 18-70 years with American Society of Anesthesiologists (ASA) physical status I-II who scheduled for laparoscopic radical hysterectomy under general anaesthesia were enrolled. Patients were randomized to receive either sufentanil (Group S) or nalbuphine (Group N) to receive either sufentanil (Group S) or nalbuphine (Group N) in the intraoperative and postoperative periods. The time to the first passage of flatus, the time to the first defecation, the time to the toleration of diet, the serum gastrin and the length of hospital stay were compared between the groups. Postoperative pain by visual analogue scale (VAS), the number of PCA(patient-controlled analgesia)s, postoperative nausea and vomiting, and dizziness were compared between the groups. Results The time to the first passage of flatus(p =0.551), the time to the first defecation(p =0.310), the time to the toleration of diet(p =0.182), the serum gastrin(p =0.397), the number of postoperative nausea(p =0.920), vomit(p =0.334) and PCA events(p =0.167) and the length of hospital stay(p =0.482) were not significantly different between the two groups. VAS scores at postoperative 6 h(p=0.008), 12 h(p =0.002) and 24 h(p =0.013) were lower in Group N than in Group S. Conclusions Compared with sufentanil, nalbuphine was not associated with improved postoperative GI dysfunction after laparoscopic surgery for gynecological malignancies, but it was associated with reduced postoperative pain. RESUMENObjetivo: comparar beneficios y complicaciones de la administracion temprana de dieta en el postoperatorio de pacientes con histerectomía abdominal. Metodología: Ensayo clínico aleatorizado, multicéntrico, participaron seis hospitales de la región centroamericana, se calculó muestra de 340 pacientes en base al número total de histerectomías en los hospitales participantes; 289 mujeres a quienes se realizó histerectomía fueron enroladas en el estudio, 155 asignadas a la intervención de dieta temprana y 134 al grupo de dieta tardía.Se realizaron tablas de frecuencias, análisis bivariado y multivariado, para la prueba de hipótesis se utilizó U de Mann Whitney. Resultados: Se presentaron características sociodemográficas similares, edad promedio Rango de 45-47 años, la presencia de ruidos intestinales fue 8 horas en promedio en el grupo de dieta temprana y 9 horas el grupo de dieta tardía, la primera evacuación se presentó a las 19.3 horas el grupo de dieta temprana y a las 27.4 horas el grupo de dieta tardía; en el grupo de dieta temprana se presentó entre las complicaciones vómitos con un RR 3.16 Conclusiones:las mujeres a quienes se les administró una dieta temprana presentan un retorno más rápido de la función intestinal, lo que puede repercutir en la mejoría post operatoria y no incrementa el aparecimiento de la distención abdominal.Palabras Clave: cuidado postoperatorio, histerectomía, dieta, motilidad gastrointestinalSUMMARYObjective: to compare benefits and complications of early administration of diet in the postoperative period of patients with abdominal hysterectomy. To shorten the time of first eating of the patients after orthopedic surgery by carrying out improvement project, a project team was set up to identify improvement themes and set improvement goals. Before starting the project activities, 102 patients with orthopedic surgery in our department were selected as the control group and the anesthesia of these patients was spinal anesthesia. Another 161 with orthopedic surgery patients were selected as the intervention group. We compared the time between returning to the ward after surgery and the time of first eating after the surgery. After the implementation of the improvement project, the average time of first eating was 92 minutes. The time of intervention group was shorter than that of the control group (P < 0.05), and the rate of intervention group patients who started to drink within 2 hours after surgery was higher than that of the control group patients (P < 0.05); There was no significant difference in postoperative gastrointestinal side effects between the two groups. Patients who returned to the ward at the end of the N class (22:00-08:00) had their first drink after a long time. The data indicate that by carrying out the improvement of nursing projects, the traditional concept of medical staff can be changed, the diet management and nursing process of patients during the operation period can be regulated. Moreover, the time of first-time eating after surgery for patients with orthopedic spinal anesthesia can be shorten, and the safety of early postoperative diet can be improved, which is good for patients to recover quickly after surgery.
