Guidelines of Diagnostics and Treatment of Acute Left-Sided Colonic Diverticulitis

Abstract: Background: The incidence of acute left-sided colonic diverticulitis (ACD) is increasing in the Western world. To improve the quality of patient care, a guideline for diagnosis and treatment of diverticulitis is needed. Methods: A multidisciplinary working group, representing experts of relevant specialties, was involved in the guideline development. A systematic literature search was conducted to collect scientific evidence on epidemiology, classification, diagnostics and treatment of diverticulitis. Literatu… Show more

“…Comparing RCD with LCD is even more difficult because most studies on RCD in Caucasians lack pre-operative radiological evaluation and are based on intra-operative findings. Hence, imaging is recommended in the diagnostic process of diverticulitis [22,35].…”

“…Treatment of a RCD patient is empirical because all guidelines for diverticular disease regard only left-sided diverticulitis [15][16][17]22]. These guidelines state that the natural disease course of LCD is usually mild and treatment is mostly conservative.…”

“…This will then determine the subsequent need for medical or surgical intervention. As evident from a number of guidelines issued in the past, there has been considerable variation in recommendations and approaches to patients with acute diverticulitis [2][3][4][5][6][7][8]. Much of the variation has been based on a weak or complete lack of evidence for which to make recommendations.…”

“…49 However, this position has been questioned in recent guidelines, given the low risk of detection of other pathologies in patients with asymptomatic disease following the acute attack. 83 Colonoscopy is the optimum approach for this if evaluation is required; however, this should be delayed to prevent the risk of perforation. Early colonoscopy has been advocated by some authors, but safety concerns and excessive patient discomfort have meant that the majority of patients are investigated after an 8-to 10-week period.…”
