BACKGROUND: This Enhanced Recovery After Surgery Guideline for postoperative care in cesarean delivery will provide best practice, evidenced-based recommendations for postoperative care with primarily a maternal focus. OBJECTIVE: The pathway process for scheduled and unscheduled cesarean delivery for this Enhanced Recovery After Surgery cesarean delivery guideline will consider time from completion of cesarean delivery until maternal hospital discharge. STUDY DESIGN: The literature search (1966e2017) used Embase and PubMed to search medical subject headings that included "Cesarean Section," "Cesarean Delivery," "Cesarean Section Delivery," and all postoperative Enhanced Recovery After Surgery items. Study selection allowed titles and abstracts to be screened by individual reviewers to identify potentially relevant articles. Metaanalyses, systematic reviews, randomized controlled studies, nonrandomized controlled studies, reviews, and case series were considered for each individual topic. Quality assessment and data analyses evaluated the quality of evidence, and recommendations were evaluated according to the Grading of Recommendations, Assessment, Development and Evaluation system as used and described in previous Enhanced Recovery After Surgery Guidelines. RESULTS: The Enhanced Recovery After Surgery cesarean delivery guideline/pathway has created a pathway for postoperative care. Specifics include sham feeding, nausea and vomiting prevention, postoperative analgesia, nutritional care, glucose control, thromboembolism prophylaxis, early mobilization, urinary drainage, and discharge counseling. A number of elements of postoperative care of women who undergo cesarean delivery are recommended, based on the evidence. CONCLUSION: As the Enhanced Recovery After Surgery cesarean delivery pathway (elements/processes) are studied, implemented, audited, evaluated, and optimized by the maternity care teams, there will be an opportunity for focused and optimized areas of care and recommendations to be further enhanced.

ERAS "Enhanced Recovery After Surgery"nin kısaltılmış hali olup, ERAS programları fonksiyonel iyileşmeyi hızlandırma ve postoperatif sonuçları düzeltme amacı ile kanıta dayalı girişimlerin birlikte kullanıldığı perioperatif protokollerdir. Bu programlar multidisipliner ve multimodal girişimleri içermektedir. Amaç hastaların cerrahi stres tepkilerini azaltmak, postoperatif morbidite ve mortalite ile hastanede kalış süresini kısaltmak ve hastaların cerrahiye ilişkin algılamalarını olumlu yönde etkilemektir. Obstetrikte ve perinatolojide ERAS protokolü deyince, sezaryen şu an için ilk ve tek akla gelen cerrahi girişimdir. ERAS derneği tarafından planlı veya plansız sezaryenden 30-60 dk. öncesinden başlayıp, annenin taburcu olmasına kadar geçen süre için (preoperatif, intraoperatif, postoperatif) sezaryen doğumda ERAS konsensus önerileri ile bir ERAS rehberi oluşturulmuştur. ERAS komponentleri; ön bilgilendirmenin sağlanmasını, iyi perioperatif beslenmeyi ve hidrasyonu, minimal invazif cerrahi teknikleri, normotermiyi sağlama çabalarını, postoperatif bulantı-kusmanın önlenmesini, idrar sondasının erken çıkarılmasını, postoperatif ağrının etkin bir şekilde tedavisini, hızlı mobilizasyonu ve multidisipiner bir ekip tarafından planlanan ve yönetilen koordine bir perioperatif bakımı kapsamaktadır.

Rates of New Persistent Opioid Use After Vaginal or Cesarean Birth Among US Women

IMPORTANCE Research has shown an association between opioid prescribing after major or minor procedures and new persistent opioid use. However, the association of opioid prescribing with persistent use among women after vaginal delivery or cesarean delivery is less clear. OBJECTIVE To assess the association between opioid prescribing administered for vaginal or cesarean delivery and rates of new persistent opioid use among women. DESIGN, SETTING, AND PARTICIPANTS This retrospective cohort study used national insurance claims data for 988 036 women from a single private payer from January 1, 2008, to December 31, 2016. Participants included reproductive age, opioid-naive women with 1 year of continuous enrollment before and after delivery. For participants with multiple births, only the first birth was included. EXPOSURES Peripartum opioid prescription (1 week before delivery to 3 days after discharge) captured by pharmacy claims, including prescription timing and size in oral morphine equivalents. Multivariable adjusted odds ratios were estimated using regression models. MAIN OUTCOMES AND MEASURES Rates of new persistent opioid use, defined as pharmacy claims for 1 or more opioid prescription 4 to 90 days after discharge and 1 or more prescription 91 to 365 days after discharge among women who filled peripartum opioid prescriptions. RESULTS In total, 308 226 deliveries were included: 195 013 (63.3%) vaginal deliveries and 113 213 (36.7%) cesarean deliveries. Participant mean (SD) age was 31.3 (5.3) years, and 70 567 (51.0%) were white patients. Peripartum opioid prescriptions were filled by 27.0% of women with vaginal deliveries and 75.7% of women with cesarean deliveries. Among them, 1.7% of those with vaginal deliveries and 2.2% with cesarean deliveries had new persistent opioid use. By contrast, among women not receiving a peripartum opioid prescription, 0.
