Guideline-Concordant Antipsychotic Use and Mortality in Schizophrenia

Abstract: Adherence to PORT pharmacological guidelines is associated with reduced mortality among patients with schizophrenia. Adoption of outcomes monitoring systems and innovative service delivery programs to improve adherence to the PORT guidelines should be considered.

“…The results on cumulative exposure to antipsychotics resembled those of Cullen et al (3), as well as those obtained in our preliminary study (2), but the effect sizes changed slightly when we also adjusted for the effect of cumulative exposure to antidepressants and benzodiazepines. In particular, the mortality related to high-dose antipsychotic use decreased when we adjusted for the effect of concomitant high-dose benzodiazepine use.…”

“…Especially in Europe, public media have widely claimed that the shorter life expectancy of patients is attributable to long-term exposure to antipsychotics and that the use of such medications should be avoided whenever possible (1). Thus far, only two published studies have described the relationship between risk of death and cumulative antipsychotic dose in representative patient populations (2,3), but those studies did not control for the effect of concomitant psychotropic medications. Another study investigated cardiac mortality and cumulative exposure to antipsychotics (4), but it included only patients who had more than one prescription for oral thioridazine, haloperidol, risperidone, or clozapine.…”

“…Nonadherence to psychopharmacological medications has been found to predict worse outcomes for psychiatric patients,12 including relapse, rehospitalization and delays in achieving remission,13,14 violence such as reported aggression and arrests,15 suicide,16 and premature death 17,18. Furthermore, a relationship has been evidenced between nonadherence to psychopharmacological medications and the economic costs of psychiatric disorders, with higher rates of nonadherence leading to higher treatment costs 1921…”

“…Although antipsychotics are the cornerstone of FES treatment 27 and reduce psychiatric symptoms and overall mortality, 28,29 they can cause cardiometabolic adverse effects 20,21 that should be prevented. 30 Unfortunately, little is known about the trajectory of cardiometabolic risk as patients progress through their illness.…”
