The aim: To analyze cases of vascular anomalies in newborns retrospectively to study their main clinical signs and to determine indications for urgent treatment. Materials and methods: A retrospective review of 281 pediatric vascular anomalies diagnosed between 2011 and 2019 was performed. The results of clinical examination, prenatal history, laboratory, sonography, and radiological data were evaluated. Results: Clinical manifestations in the newborn period documented in 170 (60.5%) patients, anomalies were visualized prenatally in 8.9% cases. Five newborns with head and neck LMs required urgent treatment, which amounted 7.9% of all newborns with LMs. The indication for urgent surgery was acute respiratory failure. Partial malformation resection with tracheostomy was performed to those patients, along with intraoperative injection of sclerotherapeutic agents and argon coagulation of residual malformation tissues. EXIT procedure was performed in one case. No correlation (p=0.2) was found between the number of skin lesions and the severity of liver lesions in children with multifocal hepatic hemangiomas, congestive cardiac failure suddenly occurs in one patient in the third week of life. Two newborns with diffuse hepatic hamangioma were manifested right after the birth with clinical signs of liver failure, hypothyreosis, and cardiac failure. Short courses of corticosteroids performed in 7 patients with KHE and Kasabach-Merritte fenomemn for thrombocytopenia correction. Conclusions: Clinical signs of vascular anomalies in neonatal period were seen in 57.5% children with vascular anomalies, and only 2.9% of them required treatment. Treatment itself is advantageous only in cases when life-threatening complications develop, such as airways obstruction, cardiac and/or liver failure, thrombocytopenia.

Клінічні прояви гемангіом печінки (ГП) коливаються від безсимптомних уражень до застійної серцевої недостатності, печінкової недостатності, гіпотиреозу і летального наслідку. Мета: аналіз власного досвіду лікування ГП у дітей. Матеріали і методи. У п'ятнадцяти пацієнтів за період 2011-2018 рр. діагностовані ГП, які розподілилися наступним чином: 2 вогнищеві, 9 множинних і 4 дифузні. У всіх пацієнтів проводили скринінг серцевої недостатності та гіпотиреозу. Середній вік встановлення діагнозу становив 52,3±43,3 дня. Для уточнення діагнозу використовували МРТ (n=5), КТ (n=10), біопсію печінки з визначенням Glut1 протеїну (n=1). Результати. Серцева недостатність ускладнила перебіг ГП у 3 (20,0%), печінкова недостатністьу 5 (33,3%), гіпотиреоз-у 3 (20,0%) пацієнтів. Лікування пропранололом проведено у 12 пацієнтів (4-з дифузною ГП та 8-з мультифокальними ГП), тривалість терапії становила від 6 до 24 місяців, у середньому 14,9±4,3 місяця. Бронхообструктивний синдром спостерігали в 1 (8,3%) випадку, що потребувало відміни лікування. Померла одна дитина з проявами прогресуючої серцевої недостатності на тлі множинних ГП, летальність становить 6,7%. У 8 дітей з мультифокальними ГП на тлі терапії пропранололом інволюція відбулася у віці 6-18 місяців. У дітей з дифузними ГП спостерігалися резидуальні вогнища діаметром до 2 см (n=2). Дітям з гіпотиреозом проводилася замісна терапія L-тироксином. Пацієнтам з вогнищевими ГП лікування не проводилось, відмічено їх інволюцію у період 9 і 15 місяців. Висновки. Гемангіомам печінки притаманний широкий спектр клінічних проявів і високий ризик важких ускладнень. Раннє лікування бета-блокаторами симптомних ГП і ретельне спостереження за безсимптомними пацієнтами можуть запобігти ускладненням і знизити летальність.

Beckwith–Wiedemann syndrome (BWS) is an overgrowth syndrome, caused by alterations in a cluster of imprinted genes located within the chromosome region 11p15.5. Common clinical features are overgrowth, macroglossia, lateralized overgrowth, abdominal wall defects, neonatal hypoglycemia and an increased risk of embryonal tumors, such as hepatoblastomas. Periodic screening for abdominal tumors is recommended. Vascular tumors are uncommon in BWS. Diffuse infantile hepatic hemangiomas (DIHHs) are rare vascular tumors with potentially lethal complications, in particular acquired consumptive hypothyroidism, high‐output cardiac failure, liver failure and abdominal compartment syndrome. We describe a 2‐month‐old patient with hallmark clinical features of BWS and confirmed a genetic diagnosis with mosaic paternal uniparental disomy of chromosome 11p15.5 (UPD[11]pat). The patient developed hepatomegaly and elevated alpha‐fetoprotein (AFP) and was therefore suspected of having a hepatoblastoma. Abdominal echo‐color Doppler and a CT‐scan allowed diagnosis of DIHHs. She was closely monitored and underwent treatment with propranolol. Oral propranolol was effective in reducing hepatic lesions without side effects. This report may suggest that vascular tumors can also be associated with BWS.
