The article discusses methodological aspects of an investigation aimed at promoting the involvement of participants as a challenge. We were interested in understanding how family health teams made sense of the child handbook -Caderneta de Saúde da Criança, and child development. The intervention-research promoted the sharing of viewpoints among researchers and providers in focal groups with the use of narratives making it possible for the participants to become key players of the proposed investigation. The implementation of the strategy promoted: 1) the greater ownership of the instrument, 2) the deepening and repositioning regarding crystalized issues in the functioning of teams, and 3 ) the validation of research results. These results indicate that there is a contribution of participatory research to the consolidation of SUS because they propose access to a common plan as methodological strategy and promote greater engagement of the teams in relation to the proposed themes.Keywords: Community-based participatory research. IntroductionIn this article, we discuss methodological aspects of a research entitled "Understanding the professional discourse about the practice of monitoring child growth and development in the Family Health Strategy" (2014), which aimed to investigate what causes the low rate of completion of the Caderneta de Saúde da Criança (CSC -Child Health Handbook) 1 and to understand teams' conceptions 2018; 22(67):1077-89of comprehensive healthcare in childhood. Inquiries into these themes became important during a study carried out in 2013 with mothers who use the services of Brazil's National Healthcare System (SUS) to follow up their children's health. Firstly, a low rate of completion of the CSC by Primary Care professionals was found, corroborating the literature 2-4 . Another finding suggested that the professionals did not give much importance to aspects of the CSC targeted at the promotion of child development 5,6 .The hypothesis of professionals' lack of familiarity with the CSC and with aspects related to child development motivated the conduction of a participatory research with teams of the Family Health Strategy.The participation guideline established by the SUS 7,8 represents a benchmark for the inclusion of the population in the making and control of health policies 9 . Thus, it has become a commitment that must pervade all health actions, from management 10 to the clinic 11 . It is a principle grounded on the recognition that the person who is the target of a care action is the protagonist of his/her own history 12 , being, therefore, the co-author of health production 13 .Thus, health work has two purposes for professionals, managers and users 12 : health production and a joint construction of the capacity for reflection and autonomy. Background According to the United Nations, it is estimated that by 2050, the number of people aged 80 years and older will have increased by 3 times. Increased longevity is often accompanied by structural and functional changes that occur throughout an individual’s lifespan. These changes are often aggravated by chronic comorbidities, adopted behaviors or lifestyles, and environmental exposure, among other factors. Some of the related outcomes are loss of muscle strength, decreased balance control, and mobility impairments, which are strongly associated with the occurrence of falls in the elderly. Despite the continued undervaluation of the importance of knowledge on fall prevention among the elderly population by primary care health professionals, several evidence-based (single or multifaceted) fall prevention programs such as the Otago Exercise Program (OEP) have demonstrated a significant reduction in the risk of falls and fall-related injuries in the elderly within community settings. Recent studies have strived to integrate technology into physical exercise programs, which is effective for adherence and overcoming barriers to exercise, as well as improving physical functioning. Objective This study aims to assess the impact of the OEP on the functionality of home-dwelling elderly using a common technological platform. Particularly, the impact on muscle strength, balance, mobility, risk of falling, the perception of fear of falling, and the perception of the elderly regarding the ease of use of technology are being examined in this study. Methods A quasi-experimental study (before and after; single group) will be conducted with male and female participants aged 65 years or older living at home in the district of Porto. Participants will be recruited through the network COLABORAR, with a minimum of 30 participants meeting the study inclusion and exclusion criteria. All participants will sign informed consent forms. The data collection instrument consists of sociodemographic and clinical variables (self-reported), functional evaluation variables, and environmental risk variables. The data collection tool integrates primary and secondary outcome variables. The primary outcome is gait (timed-up and go test; normal step). The secondary outcome variables are lower limb strength and muscle resistance (30-second chair stand test), balance (4-stage balance test), frequency of falls, functional capacity (Lawton and Brody - Portuguese version), fear of falling (Falls Efficacy Scale International - Portuguese version), usability of the technology (System Usability Scale - Portuguese version), and environmental risk variables (home fall prevention checklist for older adults). Technological solutions, such as the FallSensing Home application and Kallisto wearable device, will be used, which will allow the detection and prevention of falls. The intervention is characterized by conducting the OEP through a common technological platform 3 times a week for 8 weeks. Throughout these weeks, the participants will be followed up in person or by telephone contact by the rehabilitation nurse. Considering the COVID-19 outbreak, all guidelines from the National Health Service will be followed. The project was funded by InnoStars, in collaboration with the Local EIT Health Regional Innovation Scheme Hub of the University of Porto. Results This study was approved on October 9, 2020 by the Ethics Committee of Escola Superior de Enfermagem do Porto (ESEP). The recruitment process was meant to start in October, but due to the COVID-19 pandemic, it was suspended. We expect to restart the study by the beginning of the third quarter of 2021. Conclusions The findings of this study protocol will contribute to the design and development of future robust studies for technological tests in a clinical context. The functional variables of the study are the strength and muscle resistance of the lower limbs, assessed through the 30 Seconds Chair Stand Test (CST), and the mobility evaluated by the Timed-Up and Go Test (TUGT). These functional tests, together with the 4 Stage Balance Test (SBT) will allow evaluating the risk of fall, which will also be assessed with the application of Fall Risk Screening, available in the FallSensing Clinical App. The functional capacity, through the Lawton & Brody tool, the fear of falling, through the Falls Efficacy Scale - International (FES-I), the usability of the technology with the System Usability Scale (SUS) will also be evaluated. The Home Fall Prevention Checklist (HFPC) for older adults was selected to identify environmental risk factors. Two other technological solutions will be used - FallSensing Home App and Kallisto wearable device, which will allow the detection and prevention of falls. The intervention is characterized by the OEP with a common technological platform, conducted over 8 weeks and a frequency of 3 times a week. Throughout these weeks, the participants will be followed-up in person or by telephone contact by the rehabilitation nurse. Considering the COVID-19 outbreak, all guidelines from the National Health Service (NHS) will be followed. The project was funded by InnoStars, in collaboration with the Local EIT Health RIS Hub of the University of Porto. RESULTS This study was approved 10/09/2020 by the Ethics Committee of ESEP The recruitment process will start in October. The study results are expected to be published by the end of December.
