The recognition that certain clinical syndromes are due to secretion of hormones by 'non endocrine' tumours, has been among the major developments in endocrinology in the last decade and has forged a new link with oncology. The phenomenon has general clinical and biochemical implications. Clinicians now recognize that the accompanying metabolic derangements may be more malign than the neoplasm itself, may significantly affect prognosis, and they use the measurement of plasma hormone levels and related substances for diagnosis and management. From the biochemical point of view, the study of ectopic hormone secretion provides new insight into the state of tissue differentiation. It serves as an excellent model for the broader field of inappropriate tumour products, since the structure of most hormones is known and because recent technical advances have improved methods for their purification and assay. Malignant disease of the female reproductive organs accounts for over 11,000 deaths annually in the United States in patients over 65 years of age. Hormonal treatment is being used in some cases in conjunction with more conventional methods of treatment. Synthetic progesterone is used in the palliation of patients with advanced adenocarcinoma of the endometrium. A significant increase in the incidence of ovarian cortical stromal hyperplasia, obesity, diabetes and hypertension is found in these patients. For treatment of carcinoma of the ovary, a massive dose of an experimental progesterone agent, dimethylprogesterone (NSC-123018) is being tested in a group study. The treatment is used only for patients for whom conventional therapy has been unsuccessful. Although a few patients have shown remission, it is too early for conclusive results. Patients with advanced breast cancer and estradiol receptors in their tissue may show a favorable clinical response to hormonal therapy in 40–60% of cases. Patients lacking estrogen receptors have only a minimal chance of responding to hormonal therapy.

Paradoxical hypersecretion of human growth hormone (HGH) followed a glucose load in patients with endometrial cancer and atypical hyperplasia of the endometrium. All patients had normal fasting plasma glucose and serum HGH concentrations with a normal glucose tolerance curve. Their urinary estrogen concentrations were within the normal postmenopausal range. 6 months after abdominal total hysterectomy and bilateral oophorectomy, the glucose load was repeated. Following surgery all patients showed a normal suppression of HGH secretion during the hyperglycemic period. These findings suggest the existence of a HGH-releasing factor in the neoplastic and preneoplastic endometrial tissue. This factor might be related to the abnormal HGH responses observed.
