Growth hormone secretion elicited by GHRH, GHRP‐6 or GHRH plus GHRP‐6 in patients with microprolactinoma and macroprolactinoma before and after bromocriptine therapy

Abstract: GH release after GHRP-6 or GHRH + GHRP-6 is fully preserved in patients with microprolactinomas and does not differ before and after treatment with bromocriptine. Patients with macroprolactinoma have blunted responses of GH after GHRH and GHRP-6 and synergism is severely compromised. GH responsiveness to and synergistic interaction between GHRH and GHRP-6 recovers after shrinkage of macroprolactinoma with bromocriptine. Prolactin release stimulated by intravenous administration of GHRP-6 in healthy subjects wa… Show more

“…The mechanisms of pituitary function recovery following medical therapy may include tumour shrinkage or relate to re‐establishing hypothalamic control. Popovic et al . (1998) have shown that GH secretion in response to GH releasing peptide 6 (GHRP‐6) improved in patients with macroprolactinomas following therapy with bromocriptine for 6–24 months while the response to growth hormone releasing hormone (GHRH) was unchanged.…”

“…Furthermore, obese children provide high levels of IGF-1 and growth because of insulin, even after hypophysectomy [20,21,22]. Popovic et al [21] demonstrated fully preserved GH release after GHRP-6 or GHRH+ GHRP-6 stimulation in patients with microprolactinomas, before and after treatment. Patients with macroprolactinoma have blunted responses of GH.…”

“…It is useful to repeat the MRI 2–3 months after starting therapy and at longer intervals thereafter. Several studies have demonstrated recovery of impaired anterior pituitary function in association with tumour shrinkage 31 and ovulatory menses return in over 90% of premenopausal women. Dopamine agonists usually restore visual function to an extent similar to that produced by surgical decompression of the chiasm in macroprolactinoma patients.…”
