GRO alpha and interleukin-8 in Pneumocystis carinii or bacterial pneumonia and adult respiratory distress syndrome.

Abstract: Polymorphonuclear leukocytes (PMN) are the predominant inflammatory cells recruited in acute lung injury. This study compares the concentration of interleukin-8 (IL-8) to those of GRO alpha, both of which are CXC chemokines, in bronchoalveolar lavage fluid (BALF) in three acute pathologic states: bacterial pneumonia (BPN); adult respiratory distress syndrome (ARDS); and Pneumocystis carinii pneumonia (PCP). Levels of both IL-8 and GRO alpha were below 5 pg/ml in 16 nonsmoking volunteers who served as controls.… Show more

“…GRO family members, GRO-␣, GRO-␤, and GRO-␥, are neutrophil-specific chemokines and, as such, play a major role in the development of inflammatory responses. All three can be induced by respiratory syncytial virus in airway epithelial cells (44), and GRO-␣ was found to be present at higher concentrations than IL-8 in the bronchoalveolar fluid of adult respiratory distress syndrome patients (45). Along with these chemokines, both IL-1 and NE induce the cytokines IL-6 and oncostatin M, and this up-regulation is abrogated by A46R and A52R.…”

“…SARS sequelae such as transendothelial migration of polymorphonuclear cells into the lung tissues, multiple organ dysfunction, and acute respiratory distress syndrome have been postulated to associate with cytokine and chemokine dysregulation (29). IL-8 has been shown to be elevated in blood and alveolar spaces (30), and exhibits a positive correlation with the number of polymorphonuclear cells in bronchoalveolar fluid of patients with pneumonia and acute respiratory distress syndrome (31). The elevations of IL-8 and cytokines have been found in the plasma of SARS-CoV-infected patients (14) and can induce the hyperinnate inflammatory response due to the SARS-CoV invasion of the respiratory tract.…”

“…Clinically, BAL levels of the CXCR ligands IL-8 and KC correlate with the severity of pulmonary neutrophilia in ARDS and pneumonia (47). In this regard, we show that KC is highly expressed in heterologously infected mice, as is another CXCR2 ligand, granulocyte chemotactic protein-2.…”

“…VILI is an aseptic cause of acute respiratory distress syndrome (ARDS), a disease that kills 26-44% of those it afflicts (45,46). VILI-minimizing strategies reduce mortality in ARDS by minimizing CXCR2 signaling, reducing neutrophil recruitment and neutrophillic injury (44,47). In contrast, in cases of ARDS resulting from infectious causes such as heterologous pneumonia, the merits of preventing immunopathology must be weighed against the potentially deleterious effects of dampening the antiviral and antibacterial host defense responses.…”
