Asthma and chronic obstructive pulmonary disease (COPD) affect millions of people throughout Europe, being one of the leading causes of death in the continent. Both conditions also impose considerable morbidity on patients, adversely affecting individuals’ physical and psychological wellbeing, and their capacity to live and work normally. Asthma and COPD also impose a substantial economic burden on healthcare providers and wider society through both direct and indirect costs of care. Inhaler-delivered therapy has been central to the successful management of both conditions for several decades. Advances in device technology and understanding of the pathophysiology of both conditions (while theoretically introducing greater flexibility and responsiveness into the repertoire of inhalation therapies) have also added complexity and sometimes confusion into the task of identifying the precise combination of medication and delivery device best suited to the needs of individual patients. Recently published multinational consensus reports have set out best-practice frameworks for the management of both asthma and COPD. Presentations at the two symposia summarised in this report examined the implications of these guidelines for the treatment of both conditions. Special focus was on dry power inhalers (DPI) as a means of delivering effective treatment that combines ease of use and widespread acceptance among patients, with the potential to reduce medically-related emissions of greenhouses gases compared with pressurised metered-dose inhalers (pMDI). The authors emphasised the importance of patient partnership in determining the care plan, including the choice of both inhaler device and treatment; the benefits of regular monitoring of adherence to the treatment for both patients with asthma and COPD; and the benefits of simplicity, using one type of inhaler where possible to minimise critical errors in inhalation technique.

Abstract. Bronchial asthma (BA) refers to those diseases that significantly disrupt the adaptation of patients in society and the family, lead to temporary and permanent loss of working capacity, significant economic costs in many countries of the world. The goals of treating people with ВA in recent years have traditionally remained unchanged and consist in achieving effective symptom control and maintaining a normal level of activity. Despite the availability and accessibility of modern asthma control therapy, lack of control or partial control of ВА is, unfortunately, a common phenomenon among patients with BA. The m ain reason for this phenomenon is the low adherence of people with BA to anti-inflammatory therapy of the disease. Patients with BA often rely on drugs to relieve symptoms, but low adherence to basic therapy and the lack of timely increase in its volume at the initial signs of loss of asthma control sometimes lead to irreversible consequences. The m odern approach to the treatment of BA in adults and adolescents over 12 years of age according to the recommendations of GINA 2022 and NAEPP 2020 is to use a fixed combination of an inhaled corticosteroid with formoterol both for basic therapy and as needed, since it has a rapid and simultaneous anti-inflammatory effect and is effective by alleviating symptoms and reducing the risk of exacerbation. Global climate change is one of the most acute environmental problems facing humanity. The m ain cause of climate change is greenhouse gases produced by human activity. The use of large numbers of short-acting β2-agonist inhalers for the relief of asthma symptoms is one important component of total carbon emissions to the atmosphere, which can be reduced by patients using a fixed combination of inhaled corticosteroid and formoterol as a means of basic and symptomatic therapy.

“…This bold quality improvement project resulted in a significant and sustained reduction of SABA prescriptions, potentially reducing exacerbation rates. There may be wider benefits for society, as W ilkinson et al [ 37 ] reported that greenhouse gas emissions are three times higher for poorly controlled asthma compared to well-controlled asthma in the UK. Impressively, the excess carbon footprint due to suboptimal asthma control is equivalent to the total greenhouse gas emission of 140 000 houses in the UK between 2019 and 2020, and 90% of it is estimated to be related to SABA use.…”

Selected presentations are summarised from a wide range of topics: clinical problems, rehabilitation and chronic care, general practice and primary care, electronic/mobile health (e-health/m-health), clinical respiratory physiology, exercise and functional imaging.
