Anaesthesiologists must always prefer the safest method to minimize the risk for patients. At present, ultrasound-guided blockage of the greater occipital nerve can be performed in a safe manner. In this report, we presented our experience of ultrasound-guided blockage of the greater occipital nerve that we performed in a patient with a mass at the back of the neck who had risk of general anaesthesia because of comorbidities.

Bilateral büyük oksipital sinir bloğu günümüzde, ultrason eşliğinde güvenli bir şekilde yapılabil-mektedir. Bu olgu sunumunda boyun arkasında kitlesi bulunan; sahip olduğu ek hastalıkları nedeniyle genel anestezi yönünden riskli olan hastaya, uyguladığımız ultrason eşliğinde bilateral bü-yük oksipital sinir bloğunu sunuyoruz. 437 Giriş G ünümüzde rejyonal anestezi teknikleri her geçen gün gelişmektedir. Periferik sinir blokları hemodinamiyi minimal düzeyde etkilemekte ve hemodinamik yanıtta minimal azalmayı tolere edemeyecek olan yüksek riskli hastalarda en ideal seçenek olabilmektedir (1). Bu nedenle genel anestezi riskinin yüksek olduğu hastalarda uygulanabilecek rejyonal anestezi teknikleri daha fazla tercih edilmektedir. Kafanın arkası ve boynun inervasyonu servikal spinal sinirlerden sağlanır. Posterior skalpın büyük kısmının kutanöz inervasyonunu sağlayan büyük oksipital sinir C2'nin dorsal ramusundan oluşur. Servikal bölgede meninkslerden kalkan nosiseptif uyarılar C2 seviyesinde arka boynuzdaki konverjan nöronlarda sensitizasyon yaratmaktadır; büyük oksipital sinirin C2 kökünden çıkan dalının derin paraspinal kaslar ve suboksipital yapıların inervasyonundan sorumlu olması nedeniyle, büyük oksipital sinirin bloke edilmesi; inerve ettiği bu bölgelerden kaynaklanan uyarıların bloke olması anlamına gelir (2, 3). Bu sunuda; boyun arka bölümünde lipomu olan bir olguda anestezi riskini en aza indirmek için uyguladığımız ultrason eşliğinde büyük oksipital sinir blokajı yöntemi paylaşılacaktır.
Olgu SunumuYetmiş iki yaşında, hasta onamı alınan, vücut kitle indeksi 31,3 olan hastanın boyun arkasında kitle eksizyonu tanısı ile plastik cerrahi tarafından girişim planlandı. Hastanın boynu kısa ve boyun ekstansiyonu kısıtlı idi. Daha önceden kronik atriyum fibrilasyonu, Tip 2 diyabetes mellitus (DM Tip 2) ve yaklaşık beş yıllık kronik obstrüktif akciğer hastalığı (KOAH) ve hipertansiyon hikayesi olan, laboratuar değerlerinde herhangi bir sorunu olmayan, ASA III risk grubunda olan hastaya ultrason eşliğinde bilateral büyük oksipital sinir bloğu planlandı. Sağ el sırtından 18 gauge (G) kanül ile intravenöz damar yolu açıldı ve 10 mL kg -1 %0,9 NaCl -1 infüzyonuna başlandı. Premedikasyon amacıyla intravenöz (İV) 0,025 mg kg -1 midazolam (midazolam 5 mg/5 mL, Yunanistan) uygulanan hasta ameliyathaneye alındı.
