Granulomatous Lobular Mastitis: a Rare Chronic Inflammatory Disease of the Breast which Can Mimic Breast Carcinoma

Abstract: Granulomatous lobular mastitis is a rare chronic inflammatory disease of the breast. The differential diagnosis with malign breast disease is often not easy. In most cases a surgical biopsy is needed for correct diagnosis. Idiopathic granulomatous mastitis is an exclusion diagnosis, based on the demonstration of a characteristic histological pattern, combined with the exclusion of other possible causes of granulomatous breast lesions. There is still no generally accepted optimal treatment. If surgery forms par… Show more

“…The absence of necrosis and a predominantly neutrophylic infiltrate in the background favor a diagnosis of GM, also abundant epithelioid histiocytes in smear even in the absence of granulomas. However, the definitive diagnosis depends on the histology by core needle biopsy or excisional biopsy specimens (12)(13)(14). Pathologic appearances are breast lobules replaced and partly destroyed by noncaseating granulomatous inflammation, and giant cells, epithelioid cells, leukocytes and macrophages in background (15).…”

“…FNA cytologic diagnosis can lead to misdiagnosis of tuberculosis or carcinoma Tse et al (2003). There is neutrophylic infiltration in the field and a lot of granulomatous histiocytes but definite diagnosis can be achieved through biopsy or excisional biopsy and the result of histopathology (Lai et al, 2005;Verfaillie et al, 2006;Malycha, 2001).…”
