Granulomatous cheilitis, characterized by persistent inflammation of the lips and a granulomatous histology, is sometimes associated with Crohn’s disease and is a therapeutic challenge. Reported evidence indicates treatment with an anti-TNF agent (mainly infliximab) is the most recommended therapeutic option after failure of conventional treatments. The clinical case reported the effectiveness of ustekinumab, a monoclonal antibody against interleukins 12/23, to induce the remission of severe and recurrent granulomatous cheilitis in a patient with Crohn’s disease.

La queilitis granulomatosa de Miescher (QGM) es una entidad rara, de curso crónico y pronóstico incierto. Es más frecuente en las mujeres, comienza durante la segunda o tercera década de la vida, y consiste en la aparición de un edema labial recurrente en uno o ambos labios, que se vuelve persistente. Se considera la forma monosintomática u oligosintomática del síndrome de Melkersson-Rosenthal (SMR), cuya tríada se caracteriza por edema labial o facial recurrente, parálisis facial periférica recurrente y lengua plegada. El tratamiento constituye un desafío terapéutico, sin respuesta a corto plazo. Se presentan 7 casos con diagnóstico de QGM. Se describen la clínica y las opciones terapéuticas.

Inflammatory bowel disease (IBD) is primarily a disease of the gastrointestinal tract, though it can often affect other organ systems. These extraintestinal manifestations occur in a quarter to one-third of patients with Crohn's disease and ulcerative colitis. While musculoskeletal and dermatologic manifestations are the most common, it is also important to be cognizant of head, eye, ear, nose, and throat (HEENT) manifestations and educate IBD patients about them. Here we review the ocular manifestations in conjunction with the lesser-known but increasingly recognized ENT manifestations. Considering the lack of randomized controlled trials in treating HEENT manifestations of IBD, this review is primarily based on case reports, case series, and expert opinion with a particular focus on the newer literature supporting use of anti-TNF agents.
