“…It was first described in 1978 as granulomatous lesions on the diaper areas in adults, mostly in the older people and then referred to as “diaper-area granuloma of the elderly.” 2 Although the pathogenesis is still unclear, susceptibility to occlusion, maceration, irritant contact dermatitis, and local infection may be predisposing factors among adults, especially among older people who are bedridden 3 . Although lesions most often occur in the gluteal region, they have more rarely been described in other intertriginous folds, notably the inframammary area 4,5 . To our knowledge, this case is the first to report diffuse lesions of this type, resulting from pronounced maceration inside skin folds in the presence of severe obesity and infrequent washing, probably exacerbated by irritation from repeated use of topical antifungals.…”
