Abnormal skin pigmentation represents a common skin problem which may be congenitally determined or acquired during life. The former are discussed in Chapter [70]. Acquired pigmentary disorders encompass a broad range of diseases resulting not only in abnormal melanin pigmentation of the skin but also in skin colour change produced by a variety of other endogenous and exogenous pigments. The first part of this chapter examines the melanocyte and normal melanogenesis. The second part discusses not only acquired abnormalities of skin pigmentation resulting from disturbances in melanin pigmentation but also those disorders due to other endogenous or exogenous pigments. The third part describes acquired hypomelanosis including disorders with reduced pigmentation (hypopigmentation) and total absence of pigmentation (depigmentation). In the final part, non‐melanin pigmentation of the skin is examined.

Skin pigmentation by melanocytes is regulated by complex mechanisms including melanoblast migration, melanosome transport, melanosome transfer and melanogenesis. Melanogenesis is influenced by UV light, and endocrine, paracrine and autocrine factors. This leads to a large differential diagnosis of pigmentary disorders. Additionally, melanocytes are highly immunogenic and can be targeted by the immune system, leading to vitiligo and halo naevi. Non‐melanin pigmentation arises from endogenous sources such as haemosiderosis, carotenoderma and ochronosis or exogenous pigmentation such as metals, mediation and tattoos.

GvHD remains associated with significant morbidity and mortality despite new techniques for allogeneic stem cell transplantation (SCT), such as optimized conditioning regimens. Within the past ten years, the incidence of acute GvHD has remained unchanged and the incidence of chronic GvHD has even increased. The traditional classification of GvHD according to the time of clinical manifestation is now out-dated. Acute GvHD symptoms may even occur after 100 days; vice versa, primary chronic GvHD may already be observed one month after stem cell transplantation. The current classification introduced by the National Institutes of Health includes classic acute GvHD (up to 100 days), late-onset acute GvHD (after 100 days), as well as an overlap syndrome showing features of acute and chronic GvHD and classic chronic GvHD without any time limit. Diagnosis of GvHD of the skin remains difficult because of histological similarities to drug eruptions and viral exanthems. In this first part of the article the pathophysiology, classification, skin manifestations of acute and chronic GvHD and the histopathology will be presented. In a second part the prognosis, prophylaxis and therapy of GvHD will be discussed.
